isn't "off heading alarm" about difference between magnetic true bearing and gyro true bearing? I thought "off course alarm" is about the difference of auto pilot set value and COG.

It depends how much value you want to put usually 10 deg is ok and frequency is also depends on you ...some pre departure checklist says at every departure.
