Legend!!! I personally think he didn't say 'even the Cleaners' in a derogatory way. This program is about Frontline Health Workers, and most of the time, people only remember doctors and nurses, and people tend to forget about those other health workers working very hard behind the scenes such as cleaners, security, ambulance drivers... to make the doctors', nurses' tasks easier. Therefore, him mentioning 'even the cleaners' is a tribute to all those health workers working hard behind the scenes.
