As a "non-medical student" but a life long (60+ years) patient with great interest in all things medical, I have seen numerous cxr's and I found your presentation fascinating, confident and competant! Haha. I can imagine the Consultants of years ago questioning a student and watching the poor young fellow wanting the ground to open up and swallow them (student or Consultant!). That would make an interesting Earth scan haha

This was an excellent explanation of the basics of an xray. Many of us medical professionals have a short attention span so the fact that it was 10 mins is perfect! Love how clear, concise and thorough you were with your systemic approach. Many times as an ICU nurse we're the first ones to see the results so it's very helpful to have some background knowledge on this, thank you very much for sharing Ollie.

Loved the video. I like to make B "Bony structure and Soft tissue" and save the breathing for the "everything else" category because it's often related to the diagnosis so I save it for the end. As long as there's structure and it's repeatable then should all lead to the same clinical interpretation at the end.

I am not a doctor, nor do I have the intellect to be one! I quite simply have just been for a chest X-ray and wondered what my doctor will be looking for. Luckily, my X-ray more resembled the first one rather than the second! The way you explained everything was manageable for me to understand and was very interesting. A great wee video Ollie and a really good framework for image appraisal. Would have undoubtedly suggested this resource to undergrad radiography students studying how to systematically appraise pathological radiographic appearances although am unsure whether the image used at 4:41' represents an incorrectly viewed inverted image or situs inversus. I wouldn't want to confuse students (they are very astute and will pick up on this). Ideally, PA chest projections should be annotated by the radiographer with a PA anatomical marker (inverted 'R' or 'L'), so that when you view the image (as if you were looking at the patient in the anatomical position) the anatomical marker appears the right way round. This is usually done by computer software these days although medics used to have to flip the image. However, this depends on local practices which do vary. Some hospitals will purposefully include an AP marker so that when the image is electronically flipped, the marker appears the wrong way round indicating to doctors that the image is PA. So students are advised to exercise caution when evaluating images. Would also need to check whether the patient details were fictitious/complied with data protection. But a great resource!! I continue to refer back to this video throughout my nurse practitioner program due to the lack of matierial in this are of teaching. In my 1st Radiology class our instructor, (retired ER Physician) gave us the pneumonic: "I quit and wanna be free." I: Identification; quit: quality; and wanna be: air water bone; free: funny looking things...to this day 25 years later, I still use that pneumonic to interpret x-rays! Great video (I'm sure you're a doctor by now)...

Just a mere layperson here who’s recently had his ‘annual men’s physical.’ Two years ago, one of the girls just gave me a ring to say I needed to take 2,000 IU of vitamin D. This year, they said “doctor wants you come back to go over the results.” Umm, OK. An X-Ray was a part of the exam, so I’m getting my chops ready for whatever they’re about to explain to me. At the 4:41 mark, you show an example of a "Left middle zone pneumonia". However, the image is also abnormal in that the cardiac apex, aortic arch and gastric bubble are all on the right side. There is a "Left" marker in the usual position, which suggests this patient has dextrocardia and total situs inversus. However, it is also possible that the exam was mislabeled, and the "L" marker is on the wrong side. You would need to check the clinical history and any prior CXRs to confirm the diagnosis.

As a radiography student, it’s so important to take these x-rays properly, I see it done badly a lot and I always try to do my best because every person is different and their bodies are different. The x-rays never look this good I’ll say that.

Thank you for the video. I am in grad school for Acute Care Nurse Practitioner (ACNP) and in many facilities, we are required to preview read X-Rays and at times CT Scan prior to Radiology final interpretation. Question, do you guys have or use NPs in the UK?

As a senior attending consultant, I would have given you high marks for that presentation. I might add that one must also always note the placement of the L/R orientation marker, too. An opinion concerning the relevance of the elevated right hemidiaphragm wasn't necessarily appropriate at that point. Incidentally, the RV pacing electrode was not ideally positioned. (Dr. Pancoast had been professor of surgery at my school.)

Thanks so much for this. In medical school and during residency training, I remember using the CAPE mnemonic- Centred, Aired, Projection, Exposure, but RIPE works just fine. The ABCDE mnemonic is a nice touch and ties it all neatly. Neat, if you came up with it yourself 😊👍 Just as an add on... I use density interface too. There are 3 densities that X rays catch- Air, water, bone. Like in a Chest Xray, the heart will have a water density, the ribs will have bone density and the lung fields, air density. Assessing these densities and their interface with each other, helps us identify certain tell tale signs that help us determine pathologies. An example... Air bonchograms and Kerley B lines, even Pneumothorax and Emphysema.

Describe patient profile and then RIPE, Then go for: A: Airway B: Bones (Fractures of ribs and clavicle) C: Cardiac D: Diaphragm E: External (Breast shadow, soft tissue swelling) F: lung Fields G: Gas under left diaphragm (normal) and right diaphragm (pneumoperitoneum) H: Hilar region I : Internal (Pulmonary trunk, aorta) and hence, you won't be missing anything. P.S. please during describing, say homegeneous/heterogeneous, radio-opacity/lucency, on which lung zone (not lobe). Then during listing differentials, say mass/pneumothorax/pleural effusion/ collapse whatever is relevant. Hope this will help.
