In regards to your statement about the why the fly would symbolize death, when people die it is the fly that leaves its offspring the maggot to consume the body.

Perhaps the family of the painted woman stiffed the artist and he decided to use a little whimsy to get even !

If one were, in real life, attempt to swat the fly... the the sittler in this painting would naturally, yet abruptly turn to face the painter. Now the painter is gone, and you, the viewer, remain. So, do attempt to attempt to swat the fly, but doing so brake the curse of her impending death ? Or do you leave the fly.... and instead admire her youthful beauty of the young, wealthy woman that sits poised. She is forever young and mysterious, like the painter.
