Boy Scout leader here (also Eagle Scout, etc.) Already teach the boys the basic Scout knots, and about 20 more as well. Being able to 1. Show that these knot tying skills can be important in "the real world" And 2. A different technique for tying the square knot Ahhh... it's great

Very good. With monofilament suture materials, producing the desired tissue tension is often most easily done by putting down 2 identical throws first ("granny knot"). Then the tissue tension is adjusted correctly and a throw to create a square knot is placed. The original second throw that created the granny knot may be flipped to a flat position to receive the third (square) throw by changing tension on it from the right hand to the left. If that isn't done or can't be done, an additional throw will be needed to produce a flat square knot. With ligatures it's done differently. Braided material is easier to tie, of course, but precise tissue tension is always the goal and must be achieved. On a repair that requires several interrupted sutures, the knots should all be on the same side and aligned to visualize apposition optimally, otherwise why bother being a real surgeon.

The difference is that hand tying usually keeps tension better and allows you to be more sure where you're putting the knot because you use your finger to push the knot down. Inside the body, we generally hand tie that reason. Otherwise, for example with skin lacerations, instrument tie or hand tie comes down to your own preference. Hope that helps!

Not sure if this is what you mean - you keep the free end in the same hand all the time, but yes you switch the type of knot to get a proper square knot

is it acceptable to just slip the knots down without pulling in the opposite direction? sometimes i see surgeons do that. It creates a slip knot not a square knot?

Great question! Yes, in reality, because the sutures are small and braided, perfect square knots aren't always necessary to maintain tightness. However, if you're using sutures with more memory, slip knots become more risky. Perfecting a square knot technique is all about being consistent at all times to minimize any chance of your suture becoming loose

I mean the setup makes no sense in the first place since in real life the wound you want to stitch is at a 90° angle of you, not like your setup 0° angel (you're "below" the wound going straight up)
