He pulled the pin and threw it, the grenade hit a girl in the back and it fell to the ground, the handkerchief was still wrapped around the grenade preventing the striker lever from setting the grenade off.

1:42:09 pulling the pin does nothing but allow the spring loaded lever to flip out, if something were to also be preventing the lever from flipping out and lighting the fuse (a handkerchief wrapped around it) then it would still not go off. you can pull the pin on a grenade and hold it to aim your throw as long as you want, as long as you're also gripping the lever.
