Couldn't find it either, I just had it printed out at Fedex for around $20. The gray background in some of the pages washes out the lower case lettering though. Also a good idea to get it spiralized too.

I've only found re-printed versions so far, but it's the scanned pdf re-printed (I bought one to check it out) and makes it even worse. Haven't found an original printing yet.
