I had this done in 2007 after i was stabbed in the back three times resulting in a haemopnuemothorax. Words cannot explain how much this procedure hurts. Not just the insertion, but for hours afterwards i remember feeling phenomenal discomfort in my lungs, chest...everywhere. My whole body was just engulfed in excruciating pain despite the morphine. I couldn't sit up or lie down in any sort of comfort. I vaguely remember tears falling down my face and i was moaning. It was so bad it took my mind off the stinging wounds in my back. I was so traumatised, that three years later when i got pregnant to my partner ( not the same person who stabbed me, to be clear!!) I decided straight up to have a planned caesarean as i could not bear the thought of going through hours and hours of labour pain in case it was as bad or worse than the thoracostomy. I hope to God i never feel pain like that ever again. But im grateful to the excellent medical staff for saving my life. Thanks for reading my account of this procedure.

@Achiever Hub i was in hospital 4 days. My memory is sketchy but I'm sure it went in on the night i arrived and came out on the 4th day. Next time my ex rings me I'll confirm that and get back to you because he'll remember. But I'm pretty sure it was 4 days🙂.

@Achiever Hub oh no, I'm so sorry to hear that😢did you have spontaneous pneumothorax or an injury? Have the drs told you why it is in for so long? Please feel better soon ♥️ and don't worry, it doesn't hurt coming out. You just have to hold your breath when they take it out. Then they'll stitch you up.

Achiever Hub i was in the hospital for three weeks. Keep your chin up. Once this is over youll have a crazy story to tell. Also once the tube comes out the pain resolves so fast. Like all the weight is pulled up off your shoulders. Stay strong fam

I still remember the pain from this procedure. I had pneumonia and my lung collapsed. I remember about 10 people rushing in the room. They rolled me over and held me down. I did not receive any anesthesia and I can say this was definitely the most pain I have ever experienced in my life. Coughing while this tube was inserted was hell. Removal wasn't so painful

xHeroinx same. Mine was last year. The coughing once the tube was inserted was hell. But you gotta fight through it to re-inflate your lung.. I still have intercostal pain, do you have a similar problem? And how long ago was it?

My son is only 18 and he had this procedure last night. It was such a painful procedure that i could hear him screaming in pain inside the operating room and i couldn't help but cry listening to my son in agony. He said he was given the anesthesia and the doctor said the same, but I'm just surprised how the patient still had to go through such pain.

Just a quick note: Always and I mean ALWAYS insert your finger and inspect digitally the thorax cavity! Adhesions are always a bad surprise, but putting the tube into the lungs or in the abdomen is a disaster.

I wouldn't say always and always, because you may find yourself in the case of a costal comminutive fracture and you could insert your finger right into a sharp bone fragment or worse you could insert the fragment into the lung parenchyma

I had three chest tubes going it has been three weeks after i got discharge and it still pretty hard go walk and to breath , im feeling better now but i feel like this is going yo take a real long time to heal a 100%

What happened to you? I had a left side haemopnuemothorax after being stabbed three times in the back. The thoracostomy insertion was the worst agony I've ever felt in my entire life. For hours afterwards mt lungs and chest felt like they were on fire. I couldnt get comfortable. I had as much morphine as they could give me but was still in pain and high level of discomfort. It's the second- worst thing to ever happen to me, the stabbing was the worst..but the chest tube hurt more. How did you find it?

yeah i had pneumothorax about 5 times myself back to back but thankfully my last surgery seems to have fixed it for good the surgery was always painful and waiting in the hopsital with a tube in your chest was painful too thankfully i havent had it anymore and it seemingly fixed for good

to all the nursing students on here, i am a recovering addict. had to get this procedure done due to trauma on the lungs. i ended up almost dead because the emergency room doctor in my small town would no longer listen to me as a human being he just assumed i wanted pain meds because i was an addict in the er. i went once a week and kept getting misdianosed until one day i got lucky and got a different doctor. she was my saving grace. my lungs were filled with fluid. my oxygen levels were so low i was on the highest amount of oxygen they could give me. i was rushed to another hospital and was immediately prepped for surgery. i was on a morphine pump norcos tramadol. it didnt touch the pain. unfortunaely for me i had to take dilauded instead of the morhine because i was still in so much pain after the surgery. it was the worst time in my life. the moment they took out my chest tubes they took me off all pain meds besides norco and tramadol. i was to go home with both. before i left the doctor told me they didnt feel comfotable giving me vicodin because they didnt want me to relapse. mind you i had not been weaned. two minutes before i walked out the door the doctor took the script of norco and said all i could have was tramadol. i was in so much pain i couldnt walk. i used a walker for a month. anyways i went to the pharmacy and went to fill my script. the doctor cacelled the tramadol too and left me with nothing. i understand i was an addict, but i couldnt even feel a vicodin because my tolerance was so high. so basically the hospital kept me doped up and let me leave to go home and suffer. the pain was so bad i didnt sleep for a week straight. i had to sleep sitting up. if i got any. i cried on the way hoime to my mom and told her i didnt want to get high anymore. i really didnt. but i had no choice but to go out and buy heroin because the pain was unbearable, not to mention the pain from the opiod withdrawls was just beginning. i tried to make it thru the night but the tykeonal and ibuprofen did nothing. the heroin relived alot of thet pain but i had to use a lot and frequently just to be able to properly function. the next day i had decided i had had enough i didnt want to use and i hated the hopsital for not having a heart and making me suffer so. i take responsibilty but id rather give birth over and over again than ever have to go through that again. i tried to take my life the next night. i didnt have a job i didnt want to be a junkie anymore and the pain was so unbearable i culdnt take it. addicts are humans. yes we should feel some pain as a reminder so we dont make that choice again. we should feel a little something. but everything. i have severe ptsd and i hate going to emergency rooms. please remember we are humans just like you. please have a little compassion the next time u treat an addict. I am so glad i am alive. that was 6 years ago. i am a year clean and in college to be a Recovery Support Specialist. There is hope.

Mine was so painful. They push the tube al the way in near to may collar bone til the tube bend to my shoulder. It hurts so much. My lungs was impaled.

This was the most painful thing I've ever experienced. I had a significant amount of fluid that was impacting my heart and lungs. Pneumonia and blood clots were the cause for the mass build up of the fluid. Coughing while this is in, hurts like absolute hell! It caused some lung damage. It hurt when it was taken out too. My stomach felt like it was burning (possibly because it was empty.... I kept throwing up any time I ate something.) My shoulder and rib area felt like it was being ripped apart. I wouldn't wish this procedure on any one. To anyone who has had it recently, I hope your healing is going well. 💜

It was a very good video. you didn't however mention about , the positioning of the patient, : The patient should be supine on table with arm above the head.. Also, Incision should be 2-4 cm.

Smaller bore tube thoracostomy via a Seldinger technique is far less traumatic and/or painful. Most fluid collections/air drain effectively unless a very thick pus as in empyema is present. This technique also allows for pre-insertion aspiration to confirm presence and location of effusion.

Yes, you should. Also better to prepare the sutures before attempting to insert the tube.

depends. typically no need, esp if they are on positive pressure.

If there is effusion it is better to clamp, otherwise it is really messy. For air it is better to left open - open pneumothorax is better as a tension one.

Very nice. The anchoring suture is wrapped around the tube not also "through". Anchoring so that it is impossible for the tube to be accidentally advanced is extremely important to prevent infection.The tube is occluded during placement. As you pointed out, the instrument used for thoracostomy is passed over the inferior rib so as to avoid the intercostal vessels and nerve. Also the more anterior the placement, the more those structures are jeopardized. I would always apply an iodine ointment barrier at the puncture site before applying the dressing materials.

@@sarahsalsa2461 Yes. Seems like we all had the same expirence. After the procedure I was crippled on the bed. Literally my neck against the bed rail sideways. Could not move But it saved me ! They put these big needle in me to numb me. The needle hurt bad. And it didn’t numb anything. Just thinking about it makes me squirm. I screamed so loud on the critical care unit. Everybody could hear me. Terrible Afterwards I was in shock. My eyes and mouth stayed wide open for a while.

@@envyallison926 They did my incision/put the tube more towards my back area than my side area as shown here. I had to "sleep" (not that got any) "resting" against it all night/morning. It was awful. I remember counting down the time until it could be pulled out which in turn burned like hell lol. The fluid has built up again for me, so I may need this done again at some point 🥺 I'm honestly terrified, to be honest. As awful as the original experience is, it saved my life also. I am grateful that we are both here to type these comments. I hope you are doing so much better now! 💙🤗

Local infiltration especially if the patient is conscious so they can tolerate the procedure. Remember a patient who cannot tolerate the procedure because of sensation of pain will move a lot thereby you're not getting appropriate response for stability of getting the incision correctly.

I recently had one done for fluid that impacted my heart and lungs. They made my incision more towards the back instead of where they show here. It hurt like absolute hell! Like any procedure it has it's risks and can be traumatic, but it helped me.

I'm 41, 6'2" 180. Never had any health complications. I'm currently in the hospital with a tube in the right side of my chest. Yesterday I had a spontaneous pneumothorax. It came on while walking with my morning coffee to my desk at work. Felt like a huge ball of gas in my chest. I knew something serious had happened because it was difficult to breath. I google searched "What does a collapsed lung feel like" before leaving the office. I made it to the ER and was having the tube installed shortly after. Now, the Atrium Oasis 3600 machine is collecting air from my chest but I'm worried because of the bubbling in the view finder. The doctors don't telling me much but they think its leaking. I'm a bit scared but in good spirits. I have the tube entering about 3 inches above my right nipple, In the video here the tube is in a different location than I have. how do you feel now sir? is everything okay? I'm 23 and I had a tension pneumothorax in my left lung and in the first few days i had some air leaking in the device but later the doctor told me to walk a lot (which helped me drain a lot of fluids) and then after the air suction device was on for 3 days the lesk has stopped.

Abdulmalek Abdullah best wishes to you, I’m home how and mostly healed up. The small tube in my chest was replaced with a larger tube in my side between the ribs because it was leaking and never sealed up. I my lung actually collapsed again while I’m the hospital. They placed the large tube in and it drained a lot off. I had the “Pleurodesis” and so far so good. How are you doing? I stayed home for the month of March , out of work, to reduce my risk of catching covid19 .

yer i could do this my lung collapsed 5 times in 12 months, bollicks to pain i could not breath just get that drain in, 4 or 5th intercostal space i have valves now horrible when you cant breath pannick begging for help
