Be cautious with long term ADT. The side effects actually do occur, and some are life altering. Consider the shortest duration possible if you submit to ADT at all. Consider radiation first, check out the results and your PSA, and then decide if ADT is for you. Not a doctor, but a cancer survivor here.

I have Aggressive Prostate Cancer there doing 2 yrs Hormone Therapy and 28 radiations and the side efforts are real

Same I haven't started hdt yet but decided to go that route and then radiation. I have localized cancer just in the prostate. Any advice.

@kendallchambers8165 Hi yea I took the Radition route and unfortunately Hormonel Therapy for 2 yrs but the cancer in Prostate was Aggressive,I would go with radiation 28 treatments has side effects peeing alit Fatigue but better Then surgery good luck

Me and my doctor discussed hormone therapy for 2 yrs or less. I'm a candidate for brachytherapy followed up with external radiation. Thanks for the information and good luck to you as well. How are the side affects on hormones.

Thank you. Your video answered my qustions about why my Dr recomended 4 months of ADT. I'm not wild about the idea but I can do it if its only 4 months.

More I read and see on this hormone therapy the more I understand my misery. All 3 of my cells were outside the prostate, and all scored 9s. PET Scan says all in pelvic around prostate. Lupron @ 3 months, Abiraterone 1000mg @ day Just finished 23 of 45 radiation sessions. I'm 65 . I'm thinking now I should have taken my run with the cancer. I'm gonna lose in quality of life over the next 4-5 years on this hormone therapy. It has taken everything from me. No energy at all.😢

Watch out for the brain fog, dementia, severe depression, frailty, diabetes, bone loss, muscle wasting, permanent penile and testicular shrinkage, complete loss of libido. But the NARCO MEDICO CARTEL thinks it's ok to turn us into ZOMBIE EUNUCHS.

I came within 18 hours of chemical castration (which they euphemistically like to call "hormone therapy"). Neither my urologist nor my radiation oncologist ever suggested that brachytherapy might be an option. Via lots of research, I cancelled the hormone shot and now have an appointment to look at whether I'm a good candidate for brachytherapy. For those out there who have been told they need external beam with hormone suppression, I urge you to investigate the long term effects of chemical castration..... from what I can tell, you will never be the same. Logically, brachytherapy (either low dose permanent seeds which deliver a higher dose in the long run, or high dose which is a quick but very strong exposure) makes a lot of sense as it delivers radiation most directly to the location of the cancer. There are side effects to brachytherapy, of course. These include urinary problems (relatively short term) and also that as with any prostate radiation, surgery after radiation is possibly not possible. Please do your own research and do NOT just accept what the first doc says. A doctor is very likely to recommend the thing that they are specialized in, be it surgery, external beam radiation with castration, or brachytherapy. This is not like being treated for a broken leg where the solution is a no-brainer.... YOU MUST DO YOUR OWN RESEARCH.

@@simmomantua8910 Certainly I could be wrong. Do you think doctors are always right? My point is that I was treated as much as a customer for the expensive technology that clinic uses as a patient. I am encouraging people to not simply accept the first treatment recommendation and to do their own research. Are you disagreeing with that?

SBRT (or iMRT) + Brachy (HDR) is sometimes a suitable treatment. Go to the very best center of excellence with your MRI and biopsy.

​@Brammy007a do what your doctor says you might steer someone down the wrong path yeah I'm disagreeing with you... doctors take an oath to help people they're not there to steer you the wrong way or to make money.... yes there's nothing wrong with seeking a second opinion but your doctor's there to help you there's people that did what you did and wished they stuck with the original plan but this is a free country you can do whatever you want....

This is the exact question that has been on the back of my mind. The question had remained... do I or do I not need to be under side effects of ADT for that duration? My situation: Intermediate, Gleason 3+4 = 7 with perineural and periprostatic invasion --- ADT started first (18 months recommended), high dose brachytherapy followed by 23 low dose exposures to external radiation. Thank you very much for confirming the directive that was given me regarding this issue. Everyone wants a perfect "trifecta"... no one wants the first two side effects, but the most important one for me is mortality. I don't want the cancer coming back! The side effects for me has been mostly emotional, a few hot flashes and little interest in sex. Small price to pay for staying alive. One last comment... I can't say enough positive for the health care of London (Ontario) Heath Sciences. Thank you for this post.

I refused further ELIGARD ADT CASTRATION after the first six month shot expired due to INSUFFERABLE side effects, which still continue 8 months. My Testosterone level remains well below castrate level with little hope of ever recovering. ..Did your doctor warn you that up to twenty percent of older men NEVER RECOVER ABOVE CASTRATE LEVEL and remain castrated for life???

Consider one year or less of ADT. I understand the fear of dying, of course, but the side effects can be pretty lousy and life shortening.

Got to say .... for me lupron six months treatment was a disaster ... mentally ... almost ended it all ... but brough the psa way down ... i am 75 yrs old ... started with a gleason 9 ... psa after 6 months lupron .023 .... radiation now underway ... problem being is quality of life in my situation ... 18/36 more months of castration ..... can not vision my closing years on lupron mentally again ... what a #&?$@ing dilemma .... i will see what the psa is after radiation in 4 weeks ...

I also refused further ELIGARD ADT due to the horrific side effects. Maybe you could ask your doctor to consider Bicaludamide monotherapy after you refused ADT. BICALUDAMIDE is not quite as effective, but it does help you increase your chances of a cure, as it blocks the uptake of testosterone into cancer cells instead of completely blocking production. It's dirt cheap as a generic and doesn't have nearly as bad side effects. ...Seems that doctors only push their extremely expensive toxic ADT injections, and have no concern about your quality of life issues. .....good luck with your outcome

Been there and done that. I agree. Lupron is very effective, no doubt, but highly damaging at the same time. I got off it at one year and wish I had not taken it at all. Radiation should have helped you out a ton. Let us know and Best Wishes.

The research and discovery of testosterone's link to prostate cancer dates back to the 40's and 50's with the doctor who discovered the link receiving the Nobel Prize for Medicine in 1966. In the intervening time it would have been nice for medical science to come up with an alternative treatment for men that suffer on ADT like me, but apparently there's no incentive to do so, I guess because ADT meds work so well. Personally, I am in a recurrence of my PC from 13 years ago, and being that I am stage 4 metastatic, I have been placed on both Lupron and Zytiga for the next few years. I will add that this doctor gave me some hope though for getting off my life-killing meds earlier than I was originally told.

Already (very) out of date as it does not mention recently approved Relugolix (Orgovyx). Fewer cardiac events and quicker restoration of testosterone (and quicker dissipation of noxious side effects). Has become the go-to for intermediate risk prostate cancer (alongside radiation). (Also doesnt mention PSMA and Decipher scoring.)

I'm starting relugolix soon. RD says one of the best with fewer side affects. But wants me on it for 2 yrs and but do radiation treatment in a few months.

This video was also made before the advance of PSMA PET scans. Another factor in considering how long to stay on the drug.

Why do all these specialists downplay the negative side effects of hormonal therapy. They simply say there are side effects but never explain in detail what they are, how severe the side effects are on lifestyle, how long they might last and if any are permanent. My thought is that if they are honest with patients about the above a good percentage of patients would simply say no.

Exactly. I agree one hundred percent. They do not really tell you and the patients are often scared. Patients will do anything to get rid of the cancer. Sadly, they cause severe injuries to their bodies and often regret it. Best Wishes.

I had two years of monthly firmagon injections and 5 years out, looks like my cancer is gone but my serum testerone is 14, my libido has disappeared years ago and an erection is a far off dream. These things I was not told

Thank you for your information i have just finished my Hormone implants after 2 years i also had two months of Radiation which concluded 6 months ago. I am 73 yrs old but my Wife is only 49 yrs old. We havent had Sex for over 2 years because the Combination of Radiation and Hormone therapy has made me Clinically Castrated. Its destroyed any chance of returning to how are life was before treatment. I am eternally grateful to thr Surgeons and Staff that helped me but i would like to know where do i go from here ?
