So basically, Le Fort fracture type II is characterized by a pyramidal shape. It is a diastatic fracture that encopasses the pterygoid process and runs along the zygomaticomaxillar suture, the floor of the maxilla, over the frontomaxillar suture, and runs symmetrically on the opposite side. Although, a patient may have two different types of Le Fort fractures - one type on one side, another type on the other, which means it isn't always symmetrical. It depends on the mechanism of injury. A patient with a bilateral Le Fort fracture type II will have some, not necessarily all, certain clinical signs present: periorbital edema, periorbital echymosis, telecanthus or pseudotelecanthus, elongated "dish" face, type III malocclusion, paraesthesis of the cheeks due to the zygomatic and buccal rami of the facial nerve and infraorbital nerve lesions.

Hi Hina and Dr Teeth! I have a question regarding Lefort II fractures that are done intentionally in orthognathic surgery; When doing a Lefort II fracture to move the midface forward in cases of midface deficiency, is it possible to go in through the soft tissues in some other way than intraorally? I mean can you perhaps do a transcutaneous incision or similar? If you want to avoid an intraoral incision. Just to clarify; my question is: Can you do a Lefort II fracture through any other incision than intraoral? Thank you!
