I kindly want to ask whether it is possible to have a pre op scenario for assessment and a post op scenario for planning. If this is possible, where will you pick problems from for your care plan? Will you use problems identified from your assessment or possible post op problems?. Thank you

Not sure but in this case risk of bleeding and risk of infection can be priority care problems coz you did not assess this patient post

Hello, since you didn't score the patient you may only opt for options such as Rai may have pain from the surgical site following hernia repair Aim. Rai to have a pain score of 1 or less in a scale of 0 to 3 following nursing intervention

Then the other care problem can be risk of infection or risk of bleeding. The three can do since you did not assess this patient post procedure. They can also work even after assessing the patient post op and they have no problem
