As a pediatric intensivist, we almost always go for the cuffed ET tube.

You got the tube size formula reversed Cuffed is +3.5 uncuffed is +4

Just a tid bit if you are giving anything down the et tube u must must must flush with saline after

While that is mostly true, the purpose of the flush is to improve absorption by spreading the drug out over a larger area. If the drug has been properly diluted (2-2.5 X IV dose in many cases) a flush is not needed. There is also the method of delivery past the distal end of the ETT using a French catheter which also does not require a flush, when properly diluted.

Can you tell us the cases where a cuffed ET tube would be preferably used in pediatric pts?

Generally they aren't used very often but, perhaps in a case where they canted to maintain positive end expiratory pressure, there might be a use then.
