Here's a take: I want your runs to die by your mistakes, not random dumb shit. Therefore, I propose a rule that, as long as you don't press any gameplay inputs during a level, that level doesn't count, as long as you skip it within 5 in-game seconds of starting it. So those levels that drop a powerup on you first thing wouldn't count.

Getting a one-up from the card should be considered a power up since it is technically the only powerup that has its meaning preserved from the original game. The only way to avoid it would be to go for the mushroom/flower since they have zero effects on you.

I think these challenges would be more fun to watch, if you dont have to quit after picking up something that you shouldnt have picked up but not on purpose you know? Like when you enter a pipe and it instantly drops you onto a mushroom, you should just skip instead of qutting the run. This way it would actually be skill and not luck based. You can also implement the rule that if a level should be possible you HAVE TO try it, instead of deciding if you want to or not Just my personal opinion though!
