on Causes of PD: i don't know how you would categorize this, but, a real cause can be an external lightning strike. a cause can be combinations of causes working at the same time. an example would be an insulation rated at 180kV has a minor defect at one spot the can protect only 160kV. at time of first energization all is well, but over time the insulation ages enough to drop its protection level by 15.001kV. the minor defect will now protect at 144.999kV. now that particular spot will have the conditions of a cause of PD. this would a combination of defect and aging that each alone would be a cause.
