In the DSM V gender _dysphoria_ is defined as a disorder, with “disorder” itself defined as a condition which causes dysfunction and/or harm to the individual suffering from it. That is how psychiatric conditions are categorized. Gender dysphoria is the unpleasant sense of discomfort many people feel when their gender and physical anatomy don’t correlate, causing a disorientating feeling of something not being right with one’s own body. Because these feelings cause dysfunction and harm, it is considered a disorder which psychiatrists aim to treat and alleviate the symptoms of. It is very often comorbid with clinical depression, meaning both are often diagnosed together. Being transgender is not a disorder in the DSM V because being transgender itself doesn’t cause any harm or dysfunction to the individual and does not fit the criteria for a delusional condition or disorder. Because trans people are (painfully) aware that they are of the opposite genetic sex. They are aware of the reality of their situation, their own anatomy, and their own presumed sex chromosomes (presumed because almost nobody gets their sex chromosomes sequenced, the vast majority of people do not know for sure what chromosomes they have). Ergo, there is no delusion taking place.
