How to apply for hearing disability certificate also known as Unique Disability ID or UDID or Viklang card or viklang praman patra. Apply for UDID card should be easy but before that just check by your pure tone audiometry whether you qualify to get this disability certificate by percentage calculation for hearing disability certificate by the formula suggested in this video. Also, don't forget to see examples given at the end of this video.
Hello, I am going to discuss the unemployment and Illiteracy among the deaf community in India and How the Indian government is helping these people in getting them government jobs. How to apply for a hearing disability certificate (UDID: Unique disability ID). How to calculate the percentage of hearing disability from hearing thresholds obtained through pure tone audiometry.
About Unique Disability ID
"Unique ID for Persons with Disabilities" project is being implemented with a view of creating a National Database for PwDs, and to issue a Unique Disability Identity Card to each person with disabilities. The project will not only encourage transparency, efficiency, and ease of delivering the government benefits to the person with disabilities but also ensure uniformity. The project will also help in streamlining the tracking of the physical and financial progress of beneficiaries at all levels of the hierarchy of implementation - from village level, block level, District level, State level, and National level.

Hearing impairment-
(a) "deaf" means persons having 70 DB hearing loss in speech frequencies in both ears;
(b) "hard of hearing" means a person having 60 DB to 70 DB hearing loss in speech frequencies in both ears;
The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:
Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader
The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future
The UDID Card will also help in streamlining the tracking of the physical and financial progress of beneficiaries at all levels of the hierarchy of implementation - from village level, block level, District level, State level, and National level
Note:-
Persons with Disabilities (PwDs) have already started applying online for Disability Certificate/UDID Cards from all States/UTs.
Enrolment /Application Registration number will be generated for all the applications submitted successfully.
SMS confirmation will be sent to the applicant with the enrolment/application registration number
Applicants who do not have Disability Certificate will receive an SMS with the date and time to visit the nearest CMO office for assessment
The Disability Certificate /UDID Card will be generated online after assessment and verification
SMS confirmation will be sent to the applicant with the allocation of the UDID number
The status of the application can be checked online by enrolment/application registration number

Merely audiogram is nothing present calculation is absolete. If person loss more SNR he/she will not understand speech properly. Does it included in India ie speech score? And SNR loss?

You are right. Discrimination score is also important in clinical practice. But Pure tone audiogram sets the basic right for identifying persons for hearing disability handicap certificate.
