Was the inadequate predilation a factor predisposing to the perforation? Have we to be in a hurry to Stent the lesion whenever we see a dissection?

I don’t think you have to hurry too much if there is dissection, unless off course the vessel is jeopardized with severe ischemia. I think the reason for perforation in this case was calcium flecks not visible on angio , because there isn’t any over dilatation

Thanks sir what about pressure of prolonged balloon inflation and for how long persist inflation before second step like covered stent or reverse anticoagulation and surgery

The balloon can be inflated at low pressure for as long as the patient can tolerate , and longer time can be used with perfusion balloons to reduce ischemia (as long as 1-2 hours) .
