Theres more than just the 57 minute mark - there seems to be a discrepancy with the normal values with the co2 and hco3 ranges. The video states: CO2: 35 - 45 mmHg HCO3: 22 - 26 mEq/L But medlineplus.gov/ency/article/003855.htm states the values as - CO2 - 38 - 42 mmHg HCO3: 22 - 28 mEq/L This might seem like a nag but considering even a 0.01 would make the blood pH; although be it slight, acidic or alkaline, which may not sound much to us, but metabolically it makes all the difference. Because of the narrow ranges in pH it's helpful to have good guidelines on this information.

I definitely think you are correct still! Since the bicarbonate levels are about 6 mEq/L above the normal range, this definitely indicates a metabolic alkalosis. Because I think that the lungs are trying to compensate for this metabolic alkalosis by increasing amount of carbonic acid within the blood by breathing in more carbon dioxide. So how in the world can he state that this is a condition is "metabolic acidosis"???

I enjoy your teaching style and the information is presented in a way that can be understood by students. I would argue that the interpretation of the HCO3 at 32:35 minutes into the video does not account for hydrolysis. In actuality, a decreased level of CO2 will lead to a decreased level of HCO3. If that is the case, this patient would have a base excess of approximately 3.8 mEq/L. I would interpret this ABG as a partially compensated respiratory alkalosis.

Here is what our team of scholars said: "So, respiratory alkalosis is correct, but partial compensation only occurs in an inverse relationship between CO2 and HCO3. We know there is a problem with the respiratory system (e.g. tachypnea from an underlying disease blowing off CO2 or some other way CO2 is being diffused quickly). The body is not recognizing this in a metabolic sense, or it would attempt to preserve (or compensate) for the increasingly alkalotic conditions. Partial compensation for respiratory alkalosis occurs when an intact metabolic system (e.g. kidneys) decrease the amount of serum HCO3 to acidify the blood. If the respiratory systems intact, one could also try to decrease respirations in an attempt to hold onto CO2 and acidify the urine. I'm sure there are many other ways the body attempts to correct acid-base imbalance, but a hydrolytic relationship between CO2 and HCO3 is not one we currently know to be effective or teach to healthcare professionals. Finally, base excess is used more for metabolic disturbance, not respiratory, as it is usually HCO3 or a basic metabolite causing a disturbance and being retained in the blood, causing a base excess or alkalotic condition." We hope this helps!
