When that attack is going off is determined by what you dealt the previous hit with. Combo eating is simple process. You click a 1 click food(such as shark), then saradomin brew/other potion(you can 'combo' boost potions and restores/pray pots too, same principle), then karambwan. You can also do just shark + karambwan or shark + brew. Don't forget to restore and repot after consuming brew dose(s), as it lowers your offensive stats
