Why is being a non-smoker a risk factor for PONV? If anything I'd have thought smoking would increase the risk of PONV as smoking is unhealthy.

A possible explanation is that nicotine acts as an enzyme inducer to metabolise opiates and other drugs given as part of an anaesthetic. When their metabolism is increased, their emetic side effects are reduced.
