So that's why everyone can't stay in the lockers for too long. Also Chainsmoker seems to emit the same smog, due to it making the phobia act faster.

5:51 edit: i was wrong, turns out he was just in his line of sight for 0.5 seconds (also it was just a normal Angler) old explanation that is wrong down here. the reason you died is because Blitz attacked you, and if Blitz attacks you need to use a locker to hide. This is why Z-13 (Sebastian) says "make sure you're *out of sight* whenever it passes through." and this means that no matter where you are in the room Blitz will *see you* and k### you unless you hide in a locker.

When you get Pinkie, the lights do not flicker. Pinkie lets out a horrifying screech to alert the players that shes on the way.
