@@AllAroundPlayerGamingYep, definitely a bug. XBox One version. I reset the game and revisited the location of the first one. Sentinel was back again, so I beat it and it unlocked. Also gained a second Vengeance Reservoir amulet, but it only counts as owning one.

Happened the exact same thing. I tried checking on the first location because of the comment and i spawned again...😂😂
