Proctalgia Fugax exercises and treatment at home to relieve acute anal pain and spasm with Pelvic Floor Physiotherapist Michelle Kenway www.pelvicexercises.com.au Proctalgia Fugax involves temporary spasm of the pelvic floor muscles. Proctalgia Fugax Symptoms
*Brief episode of anal pain or lower rectal pain
*Can occur day or night
*Pain can feel very severe
*No anal pain or rectal pain in between acute episodes
*Anal pain lasts from a few seconds to minutes (always less than 20 minutes in duration)
Consult with your medical practitioner to diagnose Proctalgia Fugax and exclude other possible causes of anal pain rather than self diagnosing anal pain.
Proctalgia Fugax Causes
The causes of Proctalgia Fugax are not fully understood however this condition is thought to involve spasm of the external anal sphincter which forms part of the pelvic floor muscles in both men and women.
Proctalgia Fugax Treatment
Natural treatments for Proctalgia Fugax at home that do not involve medication include the following techniques. These Proctalgia Fugax treatment methods are directed at relieving pelvic floor muscle spasm.
1. Pelvic Muscle Spasm Release - Massage and Pressure
The first thing in terms of management is to know that Proctalgia Fugax is not a serious medical condition. Remind yourself of this as anxiety may be linked to this condition and stress does not promote muscle relaxation.
* Sit on a rolled towel to apply gentle pressure to the anus and pelvic floor muscles
* Gentle massage using a tennis ball to relieve anal muscle spasm
2. Knees to Chest Stretch ‘Happy Baby Pose’
Kaufman (1982) advocates adopting the lying down knees to chest position to relieve Proctalgia Fugax. This position is similar to the Happy Baby yoga pose. In this position, grasp the inner buttocks with each hand as if to draw them apart and expel gas. This position may apply gentle stretch to the external anal spincter to help relieve muscle spasm.
3. Hot Water Bath
Potter and Bartolo (2001) recommend taking a hot water bath to relieve anal pain with Proctalgia Fugax. Hot water bath at 40 degrees C (104 degrees Farenheit) has been recommended because this has been shown to reduce anal canal pressure. A warm shower and allowing the warm water to pass across the anus may provide similar benefits if time is limited.
4. Deep Breathing Exercises for Pelvic Floor Relaxation
Deep breathing into the belly placing the hands over the lower abdomen and breathing into the lower belly and down through the pelvic floor in the area between your sitting bones. Breathe so that you gently bulge down through the anus as you breathe in as if to pass gas.
If you have ongoing Proctalgia Fugax then see your doctor for management which may include medication (Jeyarajah et al 2010) or see a Pelvic Floor Physiotherapist for pelvic floor treatment. Pelvic Floor Physiotherapists are trained in a number of different techniques to treat and relieve pelvic floor tension and spasm.

I’ve had this for over 40 years and the hot water bottle has been my saviour , placed on coccyx,knees up. It truly is hideous. I’ve a very high pain threshold but this has me breathing like I’m giving birth . I’ve never known the name nor have any of the doctors, so I thought it was just me. Thank you 🙋🏻‍♀️❤️

It can, and does last more than 20 mins. As someone who has had it for 50 years, and been diagnosed, your help is great. Episodes can last more than 40 to 60 mins.

Rev. Wayne T. O'Leary - If you are suffering for that long then you might try, if possible, to get yourself into a hot tub of water. When I used to suffer these attacks it was almost always during the night. So, I would quickly get out of bed and get myself into my bathtub and fill it with hot, but not scalding, water. It would then take a moment or two for the pain to let up and allow me go back to bed. That having been said, I believe in my own case that some of my attacks were a consequence of eating cheese with not enough fiber.

@@carleycanton1156it will get better! You’re not alone. I recommend following a Physical Therapy or Yoga Program to overcome this. It may take weeks or months, but its worth getting your pre-proctalgia life back!

Mine last for 30mins . It's so painful and unbearable, I break out in cold sweats and my blood pressure drops at times I feel like I'm going to pass out. As soon as I feel it coming on, I take anti inflams. It's wonderful that you are bringing this horrible weird pain the ass problem to awareness.

I agree it can be stress related. I can wake with horrendous pain . I do deep breathing and I just keep telling myself to relax . It works. I’ve never felt such terrible pain. Cold sweats ,drop in BP , feeling faint. It’s a nightmare when your out in public .

This is EXACTLY what is now happening to me after I was hospitalized for Ischemic Colitis as I have a clotting blood disorder & have been on Warfarin for my entire adult life. I've never experienced this before, but since my discharge @ begining of Sept 2023, these happen all of the time & I can't even work...

I've been suffering from this for 8 years. My episodes last from 20- 60 mins. I also break out into cold sweats and feel like I'm going to pass out or vomit from the pain. Mostly severe episodes happen in the middle of the night and wake me up. I also get sciatica type pain when it happens and the pain extends to my groin area. I have been experiencing that this week. Hot water bath hasn't worked for me as I feel I'll pass out, ice packs have worked for me. However trying the happy baby poses even without modifying it and the towel massage has been life saving this week. I am very happy to have found this video. Thank you. Despite it being awful it is also nice to know how many other people this happens to and finding it helps them too.

It's spreads to my groin also, have you been checked for colon cancer just curious. I have really bad toothaches and let me tell you this pain puts a toothache to shame. I love you stranger I'm sorry we have to deal with all this

Have had for a few years but last night was 1st time I felt as if I would pass out and laid down on the tile floor and broke out into cold sweat. Next time, and I hope there is no next time, will try the rolled up towel on hard chair.

Great video. It’s onset can be completely random. Or sometimes triggered by an event. I’ve had it from everything from taking a hit in rugby, falling on my bum ice skating, peeing, pooing, ejaculating. For me it’s a blunt, jabbing pain. Warm shower and a slow index finger massage is the best cure.

Warm a rolled wet wash cloth in the microwave for 30seconds in a ziplock baggie. Place between your butt cheeks and sit on a hard surface. Slowly rocking back and forth. While deep breathing and concentrating on relaxing the muscles. The pain can be very intense. Just remember it will stop eventually.

Sitting on the rolled up towel is a great idea! I’ve found walking around for a few minutes eases the pain spasm as well

I have it for many years. The pain is very intense and unbearable. Most of the time occurs during night when I am sleeping. However for my surprise, one night I was sleeping, the pain started. I woke up. I felt very hungry. The pain was there. I went to the kitchen and started eating. As I was swallowing the food the pain started to decrease and went away. From that night, anytime the pain starts I directly go to kitchen and start to eat something. And the pain immediately goes away. I don't know the connection but for me it's an instant relief.

The towel between my bum cheeks while sitting on an unpadded chair and simultaneously box breathing worked! I'm pretty sure this is the first time I've ever been able to stop an attack before it reached it's most painful pinnacle. Thank you so much! I ended up switching out of my snug capri pants and put on my loose pajama bottoms to have the towel pressure in the right location.

Aussie Girl you have helped me so much with my neck, vaginal atrophy, and now this. I had my yearly exam with my urogynocologist. She said excellent results whatever Im doing. My bladder is also not holding urine after voiding. I gave my Aussie Girl the credit. Thank you so much😘

I just had a 1 hour episode, mild pain, but couldn't fall asleep due to it. so i finally got up, walked around the room, and sat down at the computer and it went away while watching this vid. i think sitting on this soft seat cushion did the trick.

I HAVE FOUND THAT DRINKING 1 OR 2 LARGE GLASSES OF COLD WATER IMMEDIATELY REALLY HELPS RELAX THE MUSCLES.. I WOULD RECOMMEND THIS TO ANYONE!

I've had this condition for over 30 years. The first time it happened, I passed out in a cold sweat. Over the years, I've found that using an anal douche with hot water and a hot water massage can help. There have been times when I've had to leave work and go home because of it, but by the time I got home, the pain was gone. It's one of the worst pains imaginable, and I feel sorry for anyone who has to go through it.

Ive had this probably once a year for 20 years. Completely passed out the first time....drinking a glass of water fixes the problem every time...hope it helps someone...

Episodes? I would love to have episodes. My pain is there all the time when sitting or lying, in varying degrees.

Excellent video and will try some of these. I've had it since I was in my late teens, coming up 35 years of it and so does my father and sister. My episodes can be months apart - once had probably an 8 month gap, and other times can be several times in an evening. The most dangerous episode was on a freeway riding my motorbike - had to pull to the side asap, jumped off and found a private space in the trees and quickly stripped most of my gear off as I hyperventilated and almost passed out in sweats. Funny now but not exactly hilarious then! (It was freezing too as I recall...). For me too, it seems like there is a high correlation with my level of hydration, often being "too busy" to drink much water and paying the price at night. It also sometimes happens shortly after funky times. Thankfully I have a very understanding wife! Sorry about too much detail there... 🙂Cheers and thanks - Dave

By funky times i'm assuming you mean sex. This condition has happened to me for 20 something years now and it seems to be very random, happening months or even years apart. However it does seem to happen far more often after an orgasm. When you orgasm, your sphincter clenches several times which is why I believe this is the case. Also as you mentioned, lack of hydration probably also plays a part, just like with cramps elsewhere in your body. One time it was so bad for me, it lasted for 45 minutes and progressively got worse. It was so bad near the end i could barely stand up and had to splash cold water on my face so that i wouldn't pass out. Though price to pay for some temporary pleasure.

Great video! Ive found sitting in a bucket of hot water is a good alternative when you don't have a bath! 1am and just coming to the end of an attack 😣 ovulation due any day so thanks prostaglandins ugh

It’s not funny at all but I think it’s hilarious when I get an episode while driving! Mine is related to menstruation and bowel movements.

@helenagoblet well you may not be able to use the 'Happy Baby Pose' in that case and yes I know it's not funny. Many women do report Proctalgia Fugax with their period interestingly. Cheers!

I got this pain again today which is why I looked it up on YT. The towel worked within minutes while I watched the rest of the video. My attacks last about 20 minutes, but sometimes it is longer. Now I have some ideas to shorten the pain. Thank you.

I woke up today at exactly 3:45 in the morning, and experienced sever abdominal pain connected to my anus and even if I tried to defecate the pain doesn't go away. Feels like it's being stab by a knife or some electrical shock. I tried searching on the internet and this showed up. Breathing exercise works for me. Thank you for sharing this kind of video. ☺️

I don't have this condition but due to fissures and hems I do occasionally get the spasms. The breathing exercises help me the most but it hurts in the beginning when inhaling but the exhale is amazing

A few years ago I read an article where a doctor answered the question of how to relieve this intense pain and they recommended a couple of puffs of ventolin and I do this and it actually works! I’m still not sure how though but it’s something to do with the ingredients

My PF has occurred about 4-5 times a year. Mostly at night but not always. If I feel an episode coming on I quickly drink 3-4 shots of any strong alcohol(rum or vodka or peachtree) the pain stops getting worse and often will stop completely within 10 min. Of course do not drink and drive. But keep a couple small bottles (like on plane) on hand just in case. Works 95% of the time

Just an FYI.. my PF is definitely not an anal sphincter issue.. it is deeper , more of a cramp of the colon.. very painful, has caused me to sweat, shake, and rarely almost pass out.

Another attack just now.. pretty painful but took 3 shots of rum mixed with some soda as fast as I could.. kept it from going full blown. Alcohol works not what I would prefer but it’s usually easy to find in such an emergency.

I woke to this last night, I got half way to the bathroom before I passed out with the pain. I made it to the bathroom, I was on the verge of passing out and unable to physically see whilst on the loo.

Very helpful. As you suggested, I should likely get a proper diagnosis, but this sounds exactly like symptoms I encounter from time to time. My remedy is squatting low over a bowl on the floor (not a position everyone would be comfortable in or able to do). This seems to work for me.

For me it's hugely affected by diet. I never get the spasms if I'm on keto... but if I go off keto I get them a couple times per week. When flare ups happen, get a racquetball and sit on it on the hard floor... a rolled up sock also works, but not as well.

Mine is also totally triggered by diet! I found out that I’m allergic to A1 casein protein in cows milk and sensitive to wheat (not gluten strangely 🤷‍♀️) When I do the keto diet and completely avoid cows, milk, and wheat products, I never ever have these spasms. But even if I have just a tiny bit, I end up having a spasm within an hour of eating it. I have also noticed if I get severely constipated it triggers a spasm.

I go sit on the toilet everytime this happens its like tht pressure really helps soothe it. I havent been diagnosed as im so scared. The doctor on 2 ocassions have sent me for a colonoscopy to rule out things. But its such a weird thing to try even explain.

I just had a vaginal hysterectomy with anterior and posterior repair and uterosacreal fixation May 13th. I'm getting these pains bad. But what's worse, I feel like I cannot empty my bladder fully. I am so uncomfortable because of this, its making me crazy, and it hurts. I didn't experience this prior to surgery. Will this go away over time? I had to have a catheter for a day.

HEATING PAD on medium to medium high heat fold in half put it between the butt crack, sorry I don't have the proper medical term for butt crack. Horrid and awful sharp spasming pain. Pelvic floor disorder😞

When I learned that this was a cramp, I've stopped poopoing when I get it. It makes more sense to have the poo in there to stretch out all the tissues. I usually try to do a retraction of my poop. The reverse of pooping. In my head, that should expand the area. And I do it as hard as I possibly can to really stretch out any muscles that are scramped up. Just like when your toe cramps up and you have to pull it the other way, you have to really pull hard sometimes. I do that together with deep brathing and just trying to relax my whole body. Im definitely gonna try the baby pose next time. Maybe that makes, whichever muscle it is, stretch out. Man.. I wish I someday just find the exact movement to streatch it out and stop the pain. Also, I only get if for a few minutes, but I'm reading that some people here get if for 30+ mins. That is absolutely horrible. God bless you.

I have had it for decades, strange and fortunate for me that 90 per cent of the time it hits me when I am at home, usually in the evening. Its hell if it gets you when your outside, at work or whatever. Mine usually last 20 to 40 minutes.

great hearing you talk about this, not sure if its the same but after fissure surgery and few weeks prior to it i started getting this pain. in my case after going to the toilet then stinging then causes me to spasm, pushing out and sucking in my rectal area. The pushing is a reaction to the stinging pain. This eventually causes me to eventually go into chronic pain down there. Feels like a hard ball down there. It can get really bad for hours or almost most of the day, the pain is unbelievable and struggle to relax,. how do i stop my habit of pushing as a reaction to the pain?. Pain killers work but i cant keep taking them. I find when i dont go to the toilet for a day i dont have this issue. its the stinging after going to the toilet that sets it off.

Also look up levator ani pain. I had that after colon resection and had to go to pelvic floor PT. After months of pain I saw them and it was resolved with two visits.

Really appreciate the exercises you described to help relieve an episode of Proctalgia Fugax. Do you also think that Kegel exercises done regularly may act as a preventative measure, or at least lessen Proctalgia Fugax?

I was thinking the same thing! Especially since the rectum muscles are apart of the pelvic floor muscles.. I keep searching to see if someone else has used this method to help prevent rectal spasms but can’t find anything on it! But I’m probably going to try it, cus it can’t hurt to try! If I end up using them and it helps, I’ll definitely let you know! 😊

@@tabathawesley8974 In two recent episodes I experienced of Proctalgia Fugax, I started doing Kegels during the episode. To my surprise, the length of time of each episode was cut more than half of my average time. The average time an episode lasts for me is 30 - 40 minutes, but when doing Kegels during the episode, each of the two times was cut to just 10 - 15 minutes. I have no idea is this will continue to help for any future episodes, but nothing else I've ever tried before as worked quickly like that, so I am at least hopeful. 🙂

Pickle juice and ventolin are the only thing that work for me. Can't think about doing exercises when I'm mostly passed out in pain.

i've had this problem for a few years now...I think I suffer from it around once a month and for me it lasts between 20 minutes to an hour and ranges between agony and vomiting from the pain. I will try to incorporate some of these bits into my daily routine to see if it helps - thanks

For me they always happen at night when I'm laying in bed or asleep and last around 15 mins. They're becoming more frequent now, probably it's because I live in constant stress and anxiety in vet school haha. It's so annoying having to wake up in the middle of the night to go sit on the toilet (which is what had helped me before seeing this video!). But I've noticed that I also get a tingling sensation and a bit of numbness in one of my legs when it happens. Does anybody else get this too?

Funny, mine first started a few years after vet school. I would typically have an episode in the middle of the night once every couple months. Now that I am retired I very seldom have an episode. Good luck with yours and try not to stress too much.

It started for me during my PhD. The tingling happens to me too. From looking in to it, I'm assuming it's sciatica as the nerves are getting affected in that area during the spasming as the nerve originates in back/buttocks area

@Bth2009 great question! Just check with your doctor or a pelvic floor physio first up so that you know you're dealing with pelvic floor spasm. Yes pelvic muscle spasm can happen after hysterectomy and some other forms of pelvic surgery too. The deep breathing outlined in this video is a starting point. Physio treatment for pelvic floor spasm can involve manual therapy (muscle relase internally), sometimes dilator therapy (I'll be releasing a video on this next up) and training in how to relax the pelvic floor muscles through breathing, self massage, stretching and biofeedback to know that the muscles are relaxed. I'll put together a video on this important topic within the next month too. I hope this gives you some helpful info to start with and know that this condition can be treated. All the best 🌸

I have a similar condition, the pain is horrible but it goes away as my stock is got emptied. Until then the pain will repeated every second and its horrible that I can't move for a bit due to pain. Once the pain start, I have to rush in to toilet and empty the stomach completely to avoid the pain coming back. Anyone else have such a condition? I been to many doctors, they are not able diagnosis it properly.

Mine can be so severe I cannot move or do anything but try not to faint during episodes. Agree they can last longer - can come and go over period of time. Mine stems from endometriosis. Of course you have anxiety and stress if this pain happens to you! Sorry, telling someone to relax is not super helpful. Just know it will end, so hang on. I find biting on something helpful.

Mine have been lasting for hours. It's the worst thing I've ever dealt with. One thing I've found helpful is taking a cold, wet washcloth rolled up and placed between my butt cheeks. Sounds awkward but really helps, especially at night when I'm trying to sleep

I get this in the wee hours of the morning while I'm deep asleep. The only thing that helps is getting up and sitting on my heating pad. My heating pad is ready and plugged in just because of this. I turn it on as soon as I realise its proctalgia fugax, and I sit until it goes away. If I catch it early, it lasts maybe ten minutes. If I don't catch it early, it can last 20-30 minutes. Its horrible! I worry what if it happens while I'm out during the day. I almost want to carry a small heating pad wherever I go. But its not feasible and hope it never happens. Maybe some of these other techniques may work. I'll have to try if and when it happens. Thank you!

I just watched your video. Mine started at 3-5 minutes. As I've gotten older they are never less than 50 minutes and can be as long as 65 minutes. I have valium suppositories for mine but it takes a while (30 minutes to kick in) . I just added deep breathing and this one only lasted 50 minutes. They are so damn painful and the pain is unbearable. Next time I will try the towel or tennis ball 🎾. Are there any other interventions that may help? Thank you 🙏🏼 for this video 💗🙏🏼💗

Keeping a hit wate bag under my bum helped me 90 percent relievd ...its just the small starin which i m feeling still ...so planning to keep heat under my bum as much as possible 😂😂😂😂😂

Mine last 20-45 minutes and I feel like I'm going to pass out the pain is so bad. I'll basically be doing Lamaze breathing. :( Last night I woke up at 3am out of deep sleep and it was 50 minutes before I was pain free and able to go back to sleep.

I used to get them like that and even passed out once or twice during severe episodes. I had episodes that would last over an hour. Then I read a suggestion online and it's the only thing that ever works for me. Lean forward with feet a little more than shoulder width apart. Make a fist. Reach around behind you and turn your fist so that you can contact the anal area (or near it) with your first knuckle, which provides a small surface area that is boney, which will help focus the energy of the strike. Using the knuckle and having good aim is what is important - you don’t need to hit hard.Give a couple dozen strikes to the area, changing the timing of the hits so that they are not always in the same rhythm (to more efficiently interrupt the spasms).

I have found that drinking a teaspoon of salt dissolved in a glass of water helps. Seriously. Not making this up. The pain is so excruciating that I have at times passed out. Also, before I discovered the salt-in-water treatment (Remember... this works for me: I don't know of anyone else who suffers from PF who has tried this), the episodes lasted well over an hour.

Can this pain often be coupled with severe abdominal cramping? I have ibs flare ups that are extreme cramps in my anus and my bowels/intestines that comes in waves. It feels like nothing can get out like its spasming shut but theres so much that NEEDS to come out. I'm wondering if this condition can often be coupled with IBS or if I have something else going on. I cant see a specialist until August and so im desperate for advice 😭 so much so ill talk about my ass on KZbin LOL

@Louis Wendlander yes Proctalgia Fugax can be triggered by IBS flare ups. Have you researched the low FODMAP diet for IBS for interim management before seeing a specialist? I asked doctor for salbutamol inhaler. Definitely helps shorten duration of episodes. Mine can last the full 20-30minutes. Unbearable ..

I’ve used an albuteral rescue inhaler for years for this. It was as supposed to be for exercise induced asthma but gets more use with my sudden PF attacks. It seems to decrease the intense pain and the duration of the attack. Fortunately only happens every 4-6 months.

Mine isn't really a sharp pain.. more of a very dull and uncomfortable one. It honestly drives me nuts when it comes on. The pain is localized right on 🎈knot. It can last up to 15 minutes or so, then it just goes away. Sometimes I flex my muscles down there. It hurts while I'm doing it, but then it goes away.
