Sir any plans for Srinagar surgical appointments in future? You might have come to know a few medical negligence of surgical procedure in SKIMS and govt medical College and associated hospital Srinagar which led to patient death in both the cases It will be a relief for patients struggling to find a surgeon of this calibar in various hospitals in Srinagar . I think the only solution J and K govt should appoint you in recruitment boards or board of directors of GMC and it's associated hospital than only people of J and K will stop suffering of these daily medical negligences

Dear sir Steroids like cortisol are more of a immunosuppressant, hence your logic of body’s immunity going down because steroids are also down in the period of 2-4 am is bit contradictory and infact regarding immunity if anything it should Increase because of your same logic . As an endocrinologist i can tell You that lowest cortisol levels are at around 11 pm as the published data suggests.

Steroids that are inherently produced in the body,is what one is referring to here, not the ones that are provided & substituted from outside. This doesn’t need understanding of the endocrine system alone , but is a part of body’s response to injury. Without cortisol we would die. Everyone knows about addisonian crisis. The low levels of cortisols are indeed associated with the incidence of these surgical complications. Everything cannot be written down or explained here though.You may be right about prolonged use of steroids which may compromise the inherent & natural production of these hormones in the body to bring down the immunity. There’s a phenomenon one has written about regarding sudden collapse of a patient on the table the moment one makes an incision , observed in patients on long term steroids due to no cortisol being produced instress like surgery. We all know these are hormones of proverbial “ fight or flight “ scenario You’re welcome to join our next webinar & would be delighted to clarify what is mentioned. Yes the levels drop significantly around 2am & one has observed most of these surgical emergencies landing up around this time. God bless you

Excellent revision as always. My opinion ,the trainees need to stop Saying 'sir' in each part of the sentence. This is our typical Subcontinent style. Not prevalent in Europe,UK.
