The implication with this show is that Garu does return Pucca's affection to some level, but is too awkward, shy, and antisocial to be comfortable around her and, indeed, people in general (which is why he lives alone in the woods, away from Sooga village). Pucca's enthusiastic displays of affection are something he doesn't know how to react to, and are likely the only thing he DOESN'T know how to handle. Therefore, Pucca has, despite his mutual affection for her, become the world's most frightening thing to the otherwise unshakeable Garu. He doesn't run from her because he doesn't like her, he runs from her because he DOES and is panicking because he doesn't know what to do. He's reacting to an unknown situation like a real ninja would... Retreat, regroup, and work out a strategy. However, Pucca, being, well, Pucca, never lets him get that far, so the cycle just repeats.
