Do you use centralizes on the pipe and if so how far apart are they spaced??

We don't normally. We used double jacketed wire - and so does everyone else in our area - and I can't think of a real problem that we've had with no centralizers over the many years here. There is a real reason to NOT use them - there is NO WAY to get a water level meter probe down the hole to check the water static or pumping levels.
