I'd like to point out that the fact that the patient's pain in question 6 was better when he had a desk job and is now worse beacuse he has to do more walking is extremely atypical of inflammatory back pain (IBP). IBP, as any inflammatory joint pain, is usually relieved with motion. From my experience symptomatic ankylosing spondylitis patients usually can't tolerate any prolonged periods of inactivity, e.g. sitting. For example, i'ts not unkommon for AS patients to wake up and go to the bathroom 3-4 times during one night, not because they have to, but because it lets them take a break from the prolonged period of inactivity associated with sleep (believe it or not, a patient like this might be seen first by a urologist as this pattern looks an awful lot like BPH).

Wow, really? 1 in 5 Americans smoke regularly, that's sobering! Here in Cameroon, it's relatively really rare... For example, among the people I regularly interact with, not only does none smoke, but only one of them knows/regularly interacts with someone who smokes at all. And 3 years of being in the hospital, meeting and clerking patients, I have met one single woman who smokes.

Doctor I would love to donate. but I became physically disabled and only 46 years old with very progressed Ankylosing Spondylitis at that point I had two fractures in my lumbar but apparently in the southern states radiologist don't know how to see fractures on film I kept telling them it was broken they thought I was a crazy person misdiagnosed with fibromyalgia blah blah blah which I knew I didn't have I had to figure out what was wrong with me actually moved home to Colorado my fractures were seeing plain as day unfortunately that was like 7 years later but should have been diagnosed decades prior and yes I'm a girl my onset was about age 12 so at no fault of my own I live on a very limited income was working two jobs sometimes three so I'm sorry but I cannot donate but I sure appreciate all the 411 it's too bad all doctors can't be like you thank you very much

Oh and forgot to mention was not actually diagnosed till the age of 55 years old after all my research led me to a top rheumatologist for a s kept getting diagnosis like uh DDD bulging disc herniated disc stenosis mechanical issues osteoarthritis and actually I had heard that since I was about 27 when I was in a motorcycle wreck and the doctor came in and said I expected to walk in and see somebody in their late eighties and I said well why is that? he said I can't tell you why that is and then I was on my way and that was the end of that so I guess I had all those issues that looked like mechanical problems way back then. I really wish I could donate you're an excellent doctor like I said I wish they could all be like you really appreciate all your information what got me on your channel is I was watching the series about Ankylosing Spondylitis and you were teaching a Class? case study about a guy that was like 70 that came in with A.S and fractures you were showing what fusion looked like to the others! looking at the films actually this was very useful information to me and I can't thank you enough
