Wouldn't an ABLE account be simpler and more cost-effective?

My sibling is over the qualifying amount by $23 and was denied QMB but was permitted to continue with SLMB...that may be vice versa...In any case would a simple appeal of denial be enough to gain eligibility or would she have to create a trust for a mere $23 ?

Generally Medicaid allows for a $20 disregard. You might be able to appeal it, since it's so close, but they may hold firm that it's over the limit. Then, something would need to be done, like a trust or other means, to reduce the countable income enough to qualify for QMB. The difficult part is trusts like ours charge a fee for their services. Our minimum fee is $115, which is considerably higher than the $23 your sibling is over. In cases like this, the person needs to determine if it makes financial sense to spend money to gain the additional benefits of QMB. It's a case-by-case basis.
