Typically, quality improvement is more about improving a facility policy or procedure. For instance, finding ways to decrease hospital-acquired infections. In most instances, the patients wouldn't need to be interviewed for that process. However, for a patient safety event or a sentinel event, it's possible that the patient or family/visitors would need to be interviewed if they could provide information about what they observed happening, or what their experience was during the event. Dumb question, when would any error in healthcare be acceptable? Wouldn’t “0”% errors or defects always be the standard by default? Could you give an example of when or what area of healthcare would anything but “0”% be acceptable? Thanks!

Hi I work in healthcare risk/ quality! so almost everything is over 0 because the average is more than that. We have to establish benchmarks that take into consideration the average in the nation. Just saying 0 will make almost everything fail and you can’t compete with something that doesn’t exist! I hope that makes sense 😄
