I think it originated from someone bursting out laughing while taking a drink. Something funny being told or happening unexpected during dinner or something. As the culture matured, it later just came to express shock in general. But you are correct... depending on the drink, I wouldn't spit it out. Personally, if I'm in the middle of drinking something and someone makes me laugh, I'm more the type to choke and die or risk embarrassment while drooling my drink all over myself. It's a difficult decision. In the end I don't think "he died with root beer in his lungs" would surprise my family all too much though...
