Sir, in 1st case how u can differentiate focal orchitis from testicular mass, as both have heteroechoic areas with increased vascularity ?

Excellent question. Clinical history is key, and testicular neoplasm will persist despite treatment for orchitis. There was a normal epididymis in this case as well, which is another clue as adult patients with orchitis usually have associated epididymitis. Isolated orchitis should raise suspicion.
