There is little else to explain it. Clearly Ralph had had tens of thousands worth of plastic surgery from his nose being half the size, his teeth fixed, his berry removed, etc. He’s still disgusting, but what he looks like here compared to more recent times is obvious, and he doesn’t earn enough to pay for that work.
