What is the extra leather pad for which you had inserted before tying the shoe laces again btw?

They are for loose fitting shoes. You use them to take up space when the laces won't tighten any further.

They are called kilties, you use them also to protect the leather tongue from being damaged from the laces. You can see how the tongue has indentations from the laces. Some people use kilties to prevent that. Personally I think it gives the boots character so I never use them.

I believe you are referring to the toe cap. They originated to provide extra protection in case of something being dropped on ones toes. Toe caps are common in steel toed boots and dress shoes as well. Plain toe caps, like the ones in the video, are Oxford style. Ornate toe caps with designs, mainly found on high end dress shoes, are brogues. If this is not what you are referring to please disregard.
