Great video. Here are a few pointers that are also safer for the patient and easier on the doctor: 1. Use an Ultrasound for localisation of the entry point. Saves a lot of patients from a hydropneumothorax. I've seen a lot of cases that needed an ICD insertion because they had done a blind thoracocentesis as shown here. Entry is better done under direct visualisation of the needle by ultrasound guidance. 2. Do not use a cannula as an entry needle. There's a good chance of pneumothorax while removing the stillette in a cannula. Use a wide bore introducer needle instead. It's longer which means it can reach the effusion in cases the cannula cannot, as in obese/overweight patients. It is also of a wider bore, which means the doctor doesn't have to strain as much while aspiration. 3. Attach an IV line to the other end of the three way and connect that to a urine collection bag. That way, you don't have to keep count how many times you had aspirated using the 50ml syringe and the pleural fluid can be easily measured and handled.

@makteko Жыл бұрын
What is that fluid? Sorry I am a bit ignorant to this, I only clicked because I was curious.
@speter0208 Жыл бұрын
It's called pleural effusion. There are two layers of pleura, one that lines the inner side of your chest cavity and the other the outside of your lungs. In normal conditions there is some fluid between the two layers sticking them togeter and that is how you can breathe. Some diseases such as pneumonia or lung cancer can cause overproduction of pleural fluid, which compresses the lungs causing shortness of breath. Removing the fluid helps the patient breathe better and the fluid can be examined to find the undelying cause.

The goal of these videos is for patients to get some insights and reassurance by having an idea of what happens during a procedure. This a KZbin video, they aren't gonna put a method in detail bc first of all, if this inspired someone to do a DIY they could get sued. Second it could be considered legally as them giving medical advice without patients observations and get sued over it. Third there are litteraly instructional videos for medical professionals available to keep ppl up to date. If find it quite bizarre that you don't know any of this and are in the medical field with this little common sense.

The anesthesia numbs the back, but when that needle punctures your lung you will feel it! It sucks. Stay healthy.

Puncturing the lung would not be good. That would cause a Pneumothorax. Actually he is puncturing the pleura space.

They did this and took it from the upper inner quadrant right lung and it was full of blood. I don’t know why they didn’t do this from the lower like this dr. Doesn’t water fluid sink to the lower point of the plural space?

I once had 3 litres from around one lung removed with this procedure because of broken ribs. They went in from the side though.

Doctors wouldn't risk this in the US out of fear of being sued. Insurance wont cover it and open them up to liability if causes a pneumothorax , patients in the US just suffer. 😢

I had nearly 64 oz fluid removed two years ago. The relief was amazing, there is a moment at the end where you’re left breathless for a moment. Also, one of my nurses dropped one of the fluid filled containers on the ground, poor girl 😢

I have a pleural catheter and I am draining between 800-1k ml every 36~ hours... I was in the hospital for over 6 weeks and was draining about 2200ml around the same 36hr time frame... They couldn’t find out why I am producing the fluid.. It gets so bad I can’t even move, let alone breathe.. It’s very scary.. I’m still not very steady on my feet and I get exhausted very fast... it feels so much better right after I drain, but it hurts like hell sometimes.... I wish I could find out what’s wrong and stop this fluid from building up.. I have been doing this for over 6mos with no improvement... Very frustrating...

Some red meat.. Definitely some fish.. I drink mostly water, cranberry juice, and I have 2 cups of coffee every morning along with 2 eggs.. I do have a couple other health issues but nothing that as far as the doctors are concerned would contribute to what is happening... Thanks again😬👍

Even with that, I still am seeing one other doctor from the next best hospital in Boston, and he hasn’t had anything more to add or offer, though I do agree with you that it’s always good to have another set of eyes on the job.. Lung cancer or other cancer entities can cause this. Sometimes a lot more while in chemotherapy. Another causes are heart insufficencies or pneumonia.
