Shallow breathing during sleep may be indictive of sleep apnea, so, the first step for you would be to have a discussion with your primary care physician and perhaps undergo a sleep study. Some sleep studies can be done at home if it is not convenient for you to travel to a sleep center. A few published studies have focused on the effects of inspiratory muscle training (IMT) in reducing the severity and the symptoms of OSA (obstructive sleep apnea), but first, discussion with your doctor.

Gary, not sure what you are referring to. I would suggest you reach out to the manufacturer y PN Medical directly. The dials on each side of the device inhale and exhale turn so that you can adjust the resistance. That is the only moveable piece. The mouthpiece is removeable and interchangeable with the 15mm connector that was included in the package.

I recommend you speak with your physician or therapist regarding your candidacy for respiratory muscle training. Most everyone could benefit from exercise directed at the Breathing muscles, but your specific goals will guide the best device and protocol

Well, you might cough more if you have pulmonary secretions that need to be mobilized, and if that is the case, than that is a good thing.

I thought so, also. I am one of those that coughs when I laugh, sing, talk, go outside with change in temps, etc. But it has actually been amazing and helped me not to cough so much. Only on my 2nd week-and others have noticed the improvement!

Hi Mary. I am a home health care speech pathologist and have recently acquired a patient who had a CVA (in basal ganglia) with subsequent moderate to severe dysphagia and dysarthria. He is currently NPO. He had received a few sessions of Vital Stimulation therapy while in rehab which was reportedly very beneficial for him. Unfortunately, I am not certified in VitalStim and will continue with swallowing and non-swallowing techniques/strategies (Masako, Mendelsohn, Shaker and others) which he learned while in the hospital before going to rehab. I wondered how effective training in use of The Breather was for improving swallow function after CVA. My patient presents with silent aspiration--no protective cough reflex, reduced hyolaryngeal excursion and delayed swallow reflex. I'd be very interested in hearing about any insights or experiences with using The Breather for severe dysphagia you have had.

Robert Arnold recently published some research specifically looking at benefit from combined respiratory muscle training (cRMT) using the Breather with patients recovering from stroke. The full article can be found here: onlinelibrary.wiley.com/doi/epdf/10.1002/lio2.483 Use of the Breather at a high level of resistance, may help to increase hyolaryngeal excursion. (Watts et al). Focus on a breath hold with inspiration may help with airway closure/protection, and I often add a "breath-hold" as a final rep in any given set. Finally, I believe the Breather can serve as a great pulmonary hygiene strategy for mobilization of secretions, particularly if your patient is not very mobile. I hope this is helpful.
