Thank you for your passion and wonderful ways of explaining things. I struggle in class because I understand better when I know why things happen instead of memorizing. My teachers get upset when I ask questions that they can't immediately explain, I wish they would say "I'll look into that" or something besides ignoring my question or getting upset.

Hi coach, I thank you very much for the vidoes you're doing. It's a big help especially in these difficult times. Your dedication and effort are admirable. I am in vent class and the graphics chapter has been so wordy and difficult to understand (especially because we are still learning online and don't have many videos/ pictures except for what is in our book. NVM that - I was retired , have my RRT - going to run into the fire so I'm getting my state license so I got to sit for the NBRC again! hehe, ID vent waveforms - this was the bug in my craw. its no bug no mo! I got this thx to you, Waveforms are here to stay like ipads so thanks a ton man - for setting it all straight n simple brotha! vent screens all digital - I'm down for all this automated stuff - now I got waveforms licked - y'all done good by me Joe!

Couple things. I want you to know I LOVE your cut and dry way of teaching things. I have used your videos SO MUCH to understand and then in return help my classmates. THANK YOU for that! But now I have a question, I have taken notes on this whole video and I am curious....I understand SIMV-PS, but what about just regular old SIMV-VC or SIMV-PC? I am a little confused what that would look like. Thank you!!

In my summer semester of the RT program, we are in mechanical ventilation now, and I didn't know how to break the waveforms down. Nor did I know how to interpret them, now I do. It isn't as difficult now.

Hello, Nandasiri! Absolutely, SIMV and MMV are very similar in the fact they both function off a mechanical ventilation strategy which allows the patient to breathe spontaneously, and mechanical breaths are time triggered and sometimes synchronized with spontaneous efforts. The difference is that MMV only gives time triggered mechanical breaths if the patient isn't generating the minimum minute volume (set by the machine RR and Vt). If the patient is unable to sustain the minimum minute volume, then in MMV, the vent will give one, two, three, however many breaths it needs to, up to the set RR, to make up the difference in the deficit between the patient's spontaneous minute volume and the set minimum minute volume. This makes MMV a closed loop ventilation strategy. Input from the patient influences what the ventilator does. SIMV doesn't care what the patient is doing from a spontaneous standpoint. SIMV will deliver the set Vt at the set RR regardless of the patient's spontaneous minute volume. Does that make sense? Let me know if not. Thanks for watching and asking your question.

Can you do a video on how to fix patients I:E ration, auto peeping, ventilator dysnnchrony, min ventilation, etc with scenarios in different modes? I am taking my tmc and this is what I feel like I’ll have the most issues with. I feel as if I have a basic understanding of the modes and settings but actually taking those setting and applying them to the modes to fix an issue with the patient is where I struggle

Thanks Coach Joe, I really have two stupid questions, first, I think In VC we have a set I time, because we already set the flow and Vt, am I wrong? Second, does the Pressure that I set in PSV mode limit the volume ? In other word, if I have a patient that is able to take more tidal volume spontaneously than I set, does I need to increase the PS ? Or is it fine he can take the volume despite what PS I set...

Hey Yazan. Not stupid questions at all. Yes, in VC tidal volume and flow = I time, therefore some might say the I time is set too (and it actually is on some vents). So yes, it's a fixed variable based on the set variables, vt and flow, but it does not vary every breath. Only when Vt or flow changes does I time change. Or if you add an insp pause. Your second question, No. PS does not limit volume. Let's say you're in SIMV with a set tidal volume of 450 and PS of 10. The vent gives the time triggered breaths at 450, all spontaneous breaths are augmented by PS and those spontaneous tidal volumes could be 250, 450, 640 etc. The patient dictates the size of each breath without any other limits. Hope this makes sense!

Or I can't know except the PS because the spontaneous breath is negative not positive?

quick question, can you hyperventilate with a normal ph? if so how would you adjust the vent settings?

Yes, pain, anxiety, fear, etc. can cause someone to hyperventilate. However, when this occurs the pH will not stay normal for long. I'm not sure if that's what you are asking or not. Is it?

Hi respiratory coach, just wanted to ask a career question: I’ve graduated 2012 worked for about 1yr before I had to leave the field (in 2013), I’m now finally mentally and physically ready and able to give it my all. I’m studying to sit for the TMC (Sept. 2019)...I’m just wondering in your experience are you familiar with RT’s gaining employment after a hiatus similar to mines and/or what challenges should I expect? Any information/suggestions you can provide would be most appreciated! Thank you again for all of your insight 🙏🏽 😉🤗
