To answer your question about where Gen. Grievous got his first light saber, it was given to him by Darth Tyranus, AKA former Jedi Knight Count Dooku, who taught Grievous how to fight with a light saber. It had belonged to Dooku's former master, a Jedi named Syfo Dias (not 100% sure on the spelling), who was also killed by Dooku when he turned to the Dark Side.

I kinda forget to count Yoda as anyone's specific trainer like Obi-wan and Anakin or Anakin and Ahsoka since Yoda trained mostly Younglings not Padawans. Like how Obi-wan is considered to have been trained by Qui-gon, but in Empire, his Force Ghost is talking to Yoda about how impatient he was "when you trained me", of course referring to his Youngling training, not his Padawan training. Of course, I'm mostly familiar with the movies and mildly educated in Legends, and despise the Disney Cannon. I'm really coming to realize how much I of a geek I am. Oh well. Ya gotta like something, right?

That's not the way Palpatine got disfigured. He was disfigured slowly because of the overuse and abuse of the dark side which consumed him. That's why Vader wasn't as powerful as him. Vader knew this and continually tried to control the Dark Side which is not possible.
