Nor have to have the responsibility of trying to prevent deaths of others and know its all on his shoulders as he has to currently work alone towards his goal. Sure he has Naoto in the future but in the past he currently has no one. Hes also just stuck at being a 15 year old kid. Hes not a brawler. He was a 26 year old just have a rather low life that suddenly had this huge responsibility thrust before him. His strength is his determination. While he can't fight he can stand up and deal with a ton of pain. Which isn't actually average. A typical person couldn't take a knife through the hand and keep fighting. Or the other stuff he goes through but I won't list due to spoilers. Hes also really good at getting people to work with him as well. I cant say much without going into manga spoilers. But its good to keep in mind that Takemitchy will never be Mikey or Draken when it comes to fighting. Expecting him to is bound to lead to disappointment.

So to answer the question at the start, yes, Naoto's life CAN indeed change by whatever metric it so happens to change by. However Naoto will have no recollection of it, just like takemichi, he just sorta gets put 12 years ahead, and this happens while he is in the present. A lot of people compare this show to Re:Zero and I love both! However, I feel more emotions when it comes to Tokyo Revengers because I enjoy and really care for not only Takemichi, but all the other characters. In Re:Zero, the only person I truly care for is Subaru so some of the emotional scenes that are supposed to hit don’t for me because I’m not emotionally invested in most of the characters. I hope Re:Zero fans don’t kill me for not being a fan of Emilia or Rem or any of the ladies for that matter.

Yes, it does change his life but he gets a wave of new memories when Takemichi returns to the present. That's how Naoto always knows when he comes back because feels the difference when the alteration of memories happens. by the way, Takemichi NEVER learns to fight...
