To me is not clear who gives the reward, and how users gain the reward. Do we need to meditate with the third party for rewards or Admob make this automatic when we create a Video reward ad unit?

when you create a rewarded video ad element, it comes with many methods within it listeners that allow the developers to catch differents events like if the user have seen the entire video ad and need to be rewarded. Hope my answer help you.

You offer an in-game reward like coins or an extra-life or whatever fits your game. This is what you use to incentivize players to watch the advertisement, thereby increasing your revenue.
