This is the best video about gout that I have watched … and I have watched a lot. I developed severe gout a few days after an operation to remove 2 parathyroid glands.

Missing from this informative presentation is that most gout is a direct result of the chronic episodes of oxygen deficiency that occur with sleep apnea, which is why most gout flares originate during sleep. (Diet is just a contributing factor.) Every oxygen-starved cell in the body produces excess uric acid fed into the blood. The reduced oxygen episodes also cause reduced kidney function, so removal of uric acid from the blood is slowed. And the excess of carbon dioxide over oxygen in the blood makes the blood more acidic, so its uric acid is more likely to precipitate as monosodium urate crystals. Those crystals form in almost everyone with sleep apnea, but they cause gout in about 14% of those with sleep apnea -- those who have the genetic predisposition for gout to manifest when the crystals form in a joint. This realization is important because all the life-threatening comorbidities of gout -- cardiovascular diseases, kidney disease, cancer, hypertension, type 2 diabetes -- are actually consequences of sleep apnea, leading to premature death in those whose sleep apnea goes unresolved, including those with gout. If the sleep apnea is resolved early enough in its development, the risk for these life-threatening diseases is greatly reduced. Gout sufferers ironically are fortunate because gout usually first manifests early in the development of sleep apnea, much before its life-threatening comorbidities. In other words, gout is an early warning of sleep apnea!
