We've seen it many ways. We replaced our decision logs and meeting minutes with CRAID logs. With Programs and Portfolios, we often use dependencies (and assumptions for the A as well). We've helped clients expand the concept to include Questions and Support Requests too : CRAIDSQ - but we just call them CRAID logs. The core concept is a single consolidated log - with the goal of making meetings more actionable and improve accountability.

@@echolocity Yes, I have seen CRAID too. The major problem of just using Excel for this purpose are how to display their inter-relationships to one another in a Project, and worst still, how they inter-relate within a Program Although registered and logged, linking a decision to an action, then with a risk and then a dependency (or more) becomes taxing. Hence, the use of a special database or app to fill in this tool feature gap.
