I think it's pretty simple: have only enough in your tax-deferred retirement accounts so that the RMD is no higher than the standard deduction. Everything else comes from tax-advantaged sources. Therefore, you will be in the 0% tax bracket in retirement, you won't have to care what happens to income tax rates (anything times 0 is 0), and you can avoid tax on Social Security.

I plan on leaving some money in my 401/IRA's as a hedge against LTC costs. I will do roth conversions on most of it, but I think leaving some in the IRA/401k will allow money to basically be taken out at a super reduced tax rate because medical expenses are deductible. We are paying 100k per year for my Mother's LTC.

Please don’t do that! Talk to a local trusted financial professional that can help build a plan for you. Taking lump sum distributions will skyrocket your income in that year and you will be pushed into higher tax brackets.
