I thought it was about Anxiety (any form, "classic", as i refer to it, AKA Generalized Anxiety Disorder or "Social", which is, you guessed it, Social Anxiety) when I first heard this track. And I still KINDA think it, though I know it's about Mania. It could also be used for Depression and ADHD, as people WITH ADHD tend to have a lot of negative/intrusive thoughts. Mine are rampant and almost debilitating. Yes, I've got ADHD. And I believe some form of Autism. I'd take flies over roaches any day. I can squish flies with my bare hands no problem, that doesn't gross me out too much. But cockroaches make this disgusting crunch when you squish them. Like a cracker being snapped in half. And their blood can have various colors like white, black, red, etc, it's so disgusting I'd rather set myself on fire than get that on my skin.
