It's not actually a slide, you need a pitch shifter pedal like a whammy pedal or ehx pitchfork set to double octave and you slowly lay into it while sustaining the note.

Its called a whammy pedal it can higher or lower the the guitar sounds

it takes practice but i just used a pitch harmonic and then used the whammy up to the note
