Roofing Essentials why we need good turnups on kliplok roofs

The fundamentals of roofing are not getting through to roofers here in Sydney.
Here, we have an example of a brand new roof that leaked from day #1.
And this leak stayed a mystery for a long time. It challenged a few roofers (including the original roof installer).
It appears that no roofer could solve the mystery.
We were requested to investigate and we found that the problem was due to a lack of appreciation to roofing essentials.
When roofers do not understand the fundamentals, they make changes to the roofing techniques that they were shown at roofing school - without any real knowledge of the serious implications.
We seem to have a training problem here...

Why didn't he silicone the hole ?

@billyGooding reckons the reason is that they are lazy. He is close to the mark. I can imagine that it could have gone like this... 1) The roof sheet installer got pissed off by the obstruction and the easiest solution was to cut away the turnups. He figured that the flashing installer would fix his handiwork later on. Since installers do not have silicone handy and the hole was too big for silicone. He figured that it was not that important to the roof installation. 2) The flashing installer came along and his job was to install the flashings. Not to remedy someone else's errors. He may not even have realised that there was a hole. Anyway, he did not think that it was important enough to repair (if he noticed it). 3) If the job looks good, everyone will be happy. Just hide the faults and turn it into someone else's problem. This fiction is unfortunately quite true in our building industry.

Or sometimes people do know the reason why but they're just lazy.
