I think Sam is talking to a shrink and letting out what he needs to say. This song is perfect for Sam cause he has alot of demons inside him especially emotionally and he is trying so hard since first season to heal all that grief he has and he hurts the person that loves him the most. That's what Oliver meant when he told Sam to grow up and to change or.it can kill him. He has a very fragile ego he is not able to deal with sad issues he goes away instead of taking care of the issues.
