Here are results of my MRI from 2014 . IMPRESSION, 64 year old male. 1. Chronic full-thickness tear involving the supra- and infraspinatus tendons with severe fatty muscle atrophy and significant tendon retraction. 2. Ill-defined area of full-thickness cartilage loss involving the superior aspect of the humeral head. 3. Moderate to severe acromioclavicular osteoarthritis with significant undersurface spurring. 4. Moderate subscapularis tendinopathy. 5. Moderate intra-articular long head biceps tendinopathy and ill-defined labral degeneration. My surgeon wanted to do a reverse shoulder replacement. After explaining limitations that go with the replacement I declined. For 18 months I had a lot of pain. I used my arm/shoulder as much as the pain would allow. After the 18 months of pain I became almost pain free. I have full range of motion. I do have some lack of strength directly in font of my body but everything else is fine. I'm now 73 years old and still do some farming and am a active volunteer firefighter and am pain free.

I was using a gas powered pole saw up in a tree. I pulled the rope to start it and I was at a awkward angle, I pulled the rope out away from my body and the engine kicked back jerking the rope from my hand. I heard the rotator cuff tear and the pain was excruciating. ER doctor took xrays and said it wasn't broken. I told him I heard and felt it tear to which he said "You must have pulled a muscle.

Thanks, great video! I’m here because back in May 2023 I injured my right shoulder due to a bad fall at a trampoline park. Finally got an MRI and the results show “Posterior labral tear at the chondrolabral junction & tendinosis and a low-grade partial-thickness intrasubstance tear involving the subscapularis tendon cranial fibers.” I’m a very active 25 year old who trains calisthenics and gymnastics almost every day. I’m now able to do pull ups, handstands and muscle ups again but I definitely still feel a sense of instability and discomfort in my shoulder when training. Despite being consistent with my PT exercises for 2 months I don’t feel like it’s getting much better. Calisthenics is my passion and it’s really important for me to get back to 100%. Im worried that no matter how much PT I do, it just won’t heal back to 100%. Any thoughts if this will likely require surgery?

I go to see my ortho tomorrow for a follow up and he will hopefully say it’s time for an mri. This happened to me back in 2015 as well. There are a lot of factors that go into deciding to proceed with surgery. For example what conservative treatments have been tried and how much does it effect your daily life. That being said, It is concerning that a 75% tear may not heal. Good luck and let us know how you are doing.

ONE THING FOR SURE - Get an MRI test done first before you see a physical therapist. In my case; because the PA didn't think I should need one, he thought and diagnosed the pain that I am having for a "frozen shoulder" he referred me to the physical therapy and those visits felt that it made it worst. Because after the whole almost 3 month long of the therapy, I was in so much pain and my wife scolded the PA suggested for an MRI this time. To make the story short... The result of the MRI is that I have a Partial-thickness articular sided tear of the supraspinatus tendon measuring 19 x 8 mm.

Im 34 years old, very active in sports therefore getting back to my full active routine is priority. -How long should i give as an evaluation point to see if i still feel any pain or not to decide if surgery may be a better route? -What are things i can do to help with my recovery when it comes to nutrition? e.g certain foods, supplements etc -Should i be icing every day even though there is only minimal swelling or should i leave the inflammation to help with recovery? Appreciate some guidance on this. Thank you

@rotatorcuffexpert - Great video. I have questions please: - I have a full thickness 17mm supraspinatus tendon tear but not full width. Is there a reason why doctors talk about full thickness but not whether it is full width? And may I ask, could it heal like a partial thickness tear can? I only have minor pain now, the pain was worse when I had a partial thickness tear.
@rotatorcuffexpert
@rotatorcuffexpert 8 ай бұрын
Great Question Full width vs full thickness. Full width means the entire tendon is torn, full thickness means it is torn all the way through. Hope this helps. And no i do not think a full thickness tear can heal. Glad your pain isn't too bad

@@rotatorcuffexpertUPDATE: Because I had a 17mm full thickness tear, 2 surgeons said that I needed an operation and they both advised to build the surrounding muscles to help with the rehab. So I started to do hanging 2 or 3 times a day and have kept it up for 6 months. Now I have no pain, full range of motion, play highly competitive table tennis using the injured side and hence I have decided not to have the operation. I am so confused on whether or not it will heal. I am doing physical therapy and even after 2 months still so much pain. It’s only 4mm by 3mm by 2mm. I am now debating on PRP treatment. Would love so more information on whether or not this type of tear will heal on its own. To tears repair themselves and close?? @michaelbasilio2281, I know it is confusing and I will do a video discussing that specifically. In general these types of tears are are within the tendon and so there is nothing to "close". far as Ultrasound, it is not my favorite imaging as MRI is so much better for quantifying tears.(IMO). Thanks for this video. I have a full-thickness tear of the supraspinatus tendon with 4cm of retraction, which occurred about 3 months ago. Dr. recommends surgery. But - pain has gone away and range of motion is increasing, just a little tight. Can an injury of this type heal as well? What would explain the decrease in pain and increase in range of motion?
@rotatorcuffexpert
@rotatorcuffexpert Жыл бұрын
Hey Mike, thanks for the comment. If the tear has retracted it means that it is no longer where it is supposed to be. The body will not be able to heal that part of the tendon because it is out of place. Often people feel better because the inflammation of the injury has now resolved and the remaining intact rotator cuff and other muscles around your shoulder begin to compensate for the torn part. Hope this helps and good luck!

@@rotatorcuffexpert Dear Doctor, I just went to the orthopedic doctor. Thankfully my supraspinatus tendon is intact. The source of my pain is actually some AC joint pathology. The good news is that the long term clinical prognosis is very positive. The bad news is that it is a _long term_ clinical prognosis. My pain+discomfort will likely continue for another several weeks/months. This was actually my original suspicion, but the physiotherapist I saw was quite confident it was my supraspinatus.

Thanks for sharing your insight doctor. My MRI showed (among other tears) a "high grade partial-thickness tear of supraspinatus tendon" 11 mm in total with 4mm of retraction. I'm 66, in pretty good health. I do have auto-immune disorders. There's also a moderate grade partial thickness articular sided tear of subscapularis, 12mm in width. Other inflammatory/degenerative damage were noted. I'm thinking I need this surgically repaired. I tried injections and PT but the pain is considerable and persistent. I'm feeling this isn't going to fix itself. Ideas? Thanks!

How wide is the attachment of the supraspinatus to the humeral head? Wider than 1cm? I’ve heard that prolo can gum up joints-because of the dextrose solution-can this happen with PRP? Is dextrose or anything else used in the PRP mixture? Can subdeltoid tendinitis be treated with PRP? Can repeated cortisone injections with 4 mos between weaken and cause a tear of the supraspinatus?

Gigi that is a lot of questions! The attachment size varies from person to person, but yes it is generally wider than 1 cm. As far as prolo I don't have any experience with it so I can't comment on that. There typically is no dextrose in PPR injections.

Question--can a "small partial tear" of the infraspinatus tendon get WORSE, i.e. MORE torn, if activity which caused tear (e.g.tennis) is continued? In other words, activity with the partial tear is engaged in now and without pain...but if I have a partial tear of the RC what is the likelihood of it getting aggravated and becoming a much worse tear? Do muscle tears continue to tear? I picture the rotator muscles like a rope that gets frayed with use and gradually shreds and doesn't go back to its original thickness and strength? Is this an apt analogy?

Brilzo, So when we say can it heal we are talking about real healing. Like a cut on your skin healing. It may not be 100% ( like you may have a scar from the cut) but it does heal. Now the most important thing in healing is that the majority of the tendon still has to be attached to the bone. If that is not true then it is very unlikely for it to heal.

After some research on my own, it seems I probably tore my supraspinatus, based on where the pain is and the symptoms. I don't have insurance so just toughing it out and hoping for the best. It's been a couple months. It's getting better, maybe 1% per day. BUT I've also been running a lot with it, which I'm guessing might be slowing down the healing process. I have a full tear of superspinatis with a 2cm retraction & moderate fat infiltrate. It's on my dominate rt side. I can raise my arm & get dressed, etc with mild to moderate pain only at certain angles. The surgery recovery sounds rough & I live alone & would need to hire help In thinking for the first couple of weeks. My question is what if I don't do the surgery but live with it as is? I'm 72 & in relatively good health - just osteoarthritis in most joints anyway. Is it inevitable I'll need a shoulder replacement within 5 or 10 yrs? Secondly, I've heard of the newish procedure of inserting a balloon with saline on top of the humerus which absorbs in 3-6 months but gives pain relief for 1-2 years. Maybe I get a me one every 2 years ... Saves one from big painful surgery & long recovery possibly? I'm concerned about retears & very difficult & long recovery with RC repair on my 'big' tear...

is it common to have someone that hasn't had any trama to the shoulder to have supraspinatus interstitial tearing a subscapularis interstitial tearing and degenerative labral tear at the 3:00 postion. and is this something that a 40 year old would need surgery that does a pretty labor enhance job. i've been dealing with shoulder pain for 11 months and have done therapy injection rest during all that with the pain just getting worse by the week. started out just a little like hmm what did i do to now i'm getting maybe 3 to 4 hours sleep it's a bad aching pain with sharp pain deep down all over the shoulder to now it's moving to the lower part of my neck on the left side. i have pretty good movment in my arm other then weakness and some angles i can't do with out alot of pain. 3 months ago i was on about a 2 pain lvl and now i'm pretty much at a 4 or 5 everyday most the day..

Hey Dustin, that is a lot of questions with tough answers. Much of the time the rotator cuff degenerates over time and does not need a true "injury" to have an interstitial tear. Sounds like a trip to the ortho is needed. Good luck Dustin.Please consider joining our newsletter. Just email us at dro@mybodyprotector.com and put "yes" in the subject line. Hey Doc, first of all, I love your delivery method of imparting this information. You sound like a regular dude, and I appreciate it! You could just as easily be talking about doing a brake job on a car. Lol. Anyways after about 2 1/2 years my left shoulder stopped hurting me during sleep and I gained full range of motion. No surgery. Then my right shoulder started, I retained full motion, but I have sleeping and sharp over extention pain. No insurance, so my plan is to wait it out again, but boy... it ain't easy. 👎🏼

Hi doc, when an mri doesn't list the size or thickness of the tear does that mean its small? It just says intersitial tear at the critical zone of supraspinatus. I would just like to know if its less than 50%? Do surgeons typically do surgery for an intersitial/intrasubstance tear?

Hey Batman! Look for my video on “rct partial, full, retracted. Huh?!” In general an interstitial is a tear within the tendon and less severe than less than 50% torn

@@rotatorcuffexpert ok thanks. so basically its like a partial thickness tear within the tendon(like a hole within the tendon) and not a full thickness tear because its less than 50%? The definition of full thickness is just mroe than 50%?

@joceljohnrodriguez2494, It certainly could be a rotator cuff injury. I would recommend seeing an ortho that can talk through it with you and evaluate your shoulder. Good Luck and let us know how you are doing.

Please answer this; I've not heard you talk about this here or in your others; point me there if you have. If the pain is at night and sleep quality the BIGGEST issue, what do you advise? After 3 months day pain from activities from a 10 to a hardly noticable 2 for the past month, BUT at some point during sleep, on most nights, throb will wake me up. Responds a bit to heating pad; but mostly to sitting up in bed awhile will stop it. WHY? and I would like to think healing has taken place daytime for sure ... but the night thing! I've not tried ibuprophen -- is that recommended? THANK YOU for any help!

E mc, Well there are a lot of theories on why the pain is worst at night. I think one is because we have nothing to distract us from the pain. During the day we have activities that distract us from our shoulder pain. Also the position of sleeping will put more stress on the shoulder. Lastly irritated joints that are in one position for an extended period of time- like sleeping-can become more painful. Two resources. We offer a great supplement for joint health and inflammation see our shopify store. Second I have made several videos that talk about sleep. I will link to both of these below.

@rotatorcuffexpert
@rotatorcuffexpert Жыл бұрын
A, It is promising but still there are too many unknowns. Also it is very lucrative for the injector so there are many touting miracle cures. Like the old "snake oil salesman". The most recent results show that we need more testing and anyone that receive the injection should know there is a very real chance that it will not work.

It won't heal the full thickness tear, but it can heal a partial one. I got a MRI, and the stem cell clinic said I have a full thickness tear and I could have backed out of the stem cell cost. But I had it done along with PRP, so that it can reduce the inflammation of the arthritis in the joint. Within a week I had ore than half of the pain gone, and I had to do layoff the gym and do light exercises. Now I saw a surgeon, who explained the surgery but didn't push it. I asked him what would happen if I didn't do it, he said most people come in later because with aging and use it gets worse because the shoulder joint is not working properly. Still doing PT and seeing how it goes.

My doctor read my MRI to me yesterday that my left bicep tendon isn't where it belongs and my rotater cuff has 50 percent tear. How can a bicep tendon move that much off course??
@rotatorcuffexpert
@rotatorcuffexpert 2 жыл бұрын
Hey Carol, the bicep tendon can move out of the groove where it is supposed to be if you have a subscapularis tear or if the ligament that holds it in the groove is damaged. So sorry, I hope you get it fixed soon. Check out my video on biceps tendon injury. I will link to it below.

Rototor cuff tear can lead wing scapula
@rotatorcuffexpert
@rotatorcuffexpert Жыл бұрын
Yabut, typically a rotator cuff tear does not lead to a true "winged scapula" as this is due to a nerve injury. How ever a rotator cuff tear can lead to problems with the scapula due to the muscles around the scapula compensating for the RCT.

Hey hello doc!! thanks for the information. I am a 22 yo male who regularly hits the gym . I met with a bike accident where I almost landed on my forearm. So the rotator cuff did take some force and damage. But its been around a month and the pain has really decreased but sometimes when I rotate my hand in a bowling motion or do a pull up like motion, my shoulder kills it and the place near my inside biceps below the front deltoid pain kills me too! But if i let the pain settle for a minute or two, it come backs to normal. But still my normal motion of the shoulder is limited. I explained as much as I can. Can you please let me know whats happening here. P.s. I got an x-ray no bone fractures visible.
