Anterior labrum Tate with avulsion ?! Orthopedic doesn’t recommend surgery.. yet, my line of work is physical.. I push , pull, carry patients down the stairs on a stair chair or which ever carry device is needed. I normally have discomfort at night or certain maneuvers. Physical therapy hasn’t helped. I will get a second opinion.

I am 40 years old professional Jiu-Jitsu guy. I have superior labor injury with Supraspinstus and infrasoanatis tear and AC joint osteoarthritis. He offered biceps tenodisis. I am not sure why I can not push and press up and it feels unstable during wrestling. I am not sure what should I do to comeback to actively I love

I just had a SLAP tear surgery with bicep tenodesis. It's been almost three weeks, my surgeon told me I don't need to wear my immobilizer anymore so I'm moving my shoulder more, but I still can't lift my arm very high. But I feel good about my recovery.

Thank you for this video! I'm nearly 3 weeks out from my surgery SAD/DCE, biceps tendonesis, debridement and manipulation for frozen shoulder. Your comments on comparison were just what I needed. I've been searching around every day for more info. I didn't realize going into this surgery how much pain and recovery I'd need. I thought I'd be one of those back to work in 1-2 weeks. Im still in pain, and my elbow still hurts too. I have a desk job, but its constant, fast-paced computer work in a high stress environment. I feel badly I misjudged and will be out at least 2 months, per my doctor now. Thanks again for this video.

I had bicep tenodesis surgery to repair a slap tear. During recovery, is it bad to clench muscles? I’m asking because constipation is rough being on the pain meds and I have found myself clenching a bit with my surgically repaired arm. Just worried I may be setting myself back in the recovery process.

MC, I would recommend getting in touch with your doc for some meds to help prevent the constipation. I have not seen an injury because of clenching, however I could see might be possible

Hi @trendtrader8074. It depends on the surgeon and the individual. Typically it is between 7mm and 15 mm. In the old days it was more through an open technique.

Nothing related to this video, but I had shoulder surgery done in February this year. If I were to out of interest email you a couple of the arthroscopic photos from the surgery, would you (from the images shown) comment on whether the articular cartilage (humerus/glenoid) looks normal? Obviously, these are only photos so I understand you can only comment on what the photo shows. My post op notes say the humerus and glenoid articular cartilage as "normal". Id be interested in your thoughts 🙂

what is the length of the subcapularis tendon body before it turns into musculotendinous, is it at the glenohumeral joint/glenoid labrum level that is the cutoff? basically its pure tendon that covers up the humeral head? Some of the sagittal images look like it's 50 percent muscle and 50 percent tendon and it looks close to the humeral head but i wonder what is the cutoff for that like at the level of the distal clavicle? I thought it was suppose to be beyond the pure tendon point of the medial edge of the humeral head that it becomes 50 percent muscle and 50 percent tendon?

And Arthroscopically since you can see the upper half of the subscap only do you see it's whole length of the tendon or up to where from the insertion?

You can see about half of the subscap arthroscopically. You can see most of the tendon portion of the upper half. These are all variable depending on the persons exact anatomy. These are very specific questions are you a radiologist?

Usually the labrum is shown just as a triangle at the corner in coronal and axial on MRI or textbook images so do those perspectives not show the circumferential labrum? Similar thing with the superior glenohumeral ligament and middle glenohumeral ligament where on axial or coronal they are shown partially at one side but then inferior glenohumeral ligament is shown complete from glenoid to humerus, so is the full length not shown MRI?

Yes the labrum as well as the GH ligaments are seen on MRI. Better seen with MRI arthrogram. Often times if we are looking for the rotator cuff tear in an individual who is a little older we will just get an MRI and not an arthrogram. Therefore the labrum and ligaments are not as easily seen. Hope this helps

Yes I found this real interesting about tendon repair (at least as far as my procedure & probably most). I assumed the tendon would just get anchored to the bone as in an ACL or UCL but in this case my surgeon indicated they would be trying to reestablish the enthesis (btw GT & SST) and the anchors would become vestigial (hopefully). Hence remaining fairly immobile for the first few weeks post op. Always something new to learn I guess!

@tondog9070. Glad I could help. And actually the anchors we use for the ACL and UCL have this same concept. If the tendon/ligament does not heal to bone the repair will eventually fail.

Wish my surgeon explained things this clearly it would make deciding to go ahead or not easier. I have a full thickness tear of the supraspinatus with some retraction. I get the impression from this that I could’ve made the decision to have surgery sooner rather than deliberating for months. I’ve never had categories explained like this.

My Regeneten patch failed miserably. Not only did it disintegrate inside the shoulder, the allegedly biodegradable staples were floating around, intact, doing more damage than the original accident. The repaired supraspinatus was lost, and when it went, it took my infraspinatus tendon with it. By the way, my surgeon kept telling me that my increased/non-resolved pain was due to the healing process when it was actually a failed repair. Had to demand an MRI at six months and that is when it was revealed that his repair failed. Two months later, another surgeon had to clean up that mess at which point the concern about humeral head migration came into play because the first surgeon also took down my CA ligament and now my shoulder is unstable. Trying a Stryker In-Space balloon in 10 days to see if I can retrain the remaining two rotator cuff tendons and my deltoids to keep my shoulder centered. Still baffles me that despite my profession, he still took down the CA ligament, adding 20-30% more force to the joint to keep it centered. I am a pro sports photographer and carry those large white lenses you see on the sidelines. If this spacer doesn’t work, my career is likely over. Not because of the original accident, but because of the damage done by the first surgeon going old school on me and thinking a collagen patch would be helpful. Went in with a partial thickness tear on one tendon and now I’m missing two complete tendons, have my bicep tendon tenodesis, an unstable shoulder and constant pain when I work from what appears to be bone on bone contact. Bottom line, that patch may work…but not for a complete reconstruction like I had apparently. If I only knew what I know now, no scalpel would have gotten anywhere near my shoulder because I am worse off today than when I first had surgery in 2022.

@@rotatorcuffexpert Thank you. It was supposed to have been today but an RN at my insurance company deemed a subacromial balloon spacer “experimental” and denied my surgery two days ago. Apparently extended use of narcotics or a total reverse shoulder replacement is preferable to them, or they want me simply quit my profession as a sports photographer. None of these options are palatable which just increases my disdain for what our “healthcare” system has done to those in need. That spacer was my last, minimally invasive shot to overcome the chronic pain. I cannot afford to pay out of pocket for the cost of the procedure so they have really put me in a bad spot just as demand for my images has continued to peak. Spent the entire summer traveling and shooting up to 12 hour days but only did that on low-dose oxycodone. Apparently my photo career was over at the moment of impact 2 years ago but it has taken this long for me to begin to accept that fate. Now, at 60 years of age, I need to try to find a new career, or simply crawl back under the covers and fall into a deep depression.

I was sent to pt the day after surgery then 2 days a week after, at about 4 weeks a different therapist quickly walked my arm above my shoulder and I believe she tore something back loose.

I was given celebrex and have hypertension my blood pressure at my post op visit was 200/100 and wasn't told that could be the problem.

Does the tendon actually grow back to the bone or does it just hang from the suture strings? Another question do you get phantom pains from having the tendon removed?

Hi James, The tendon has to heal down to the bone or the sutures will break eventually. I have never heard of phantom pains after removing the tendon.

I was diagnosed 11 1/2 months after shoulder replacement with C Acnes infection. Going to see infectious disease doctor this week. Was already told by my surgeon that he will have to chip out all the hardware and replace with spacer for 3 months. Then do a reverse total shoulder replacement. Really not looking forward for all that extra pain. But in constant pain now so hopefully it will all end.

I have a concern. I am 10 weeks post-op ( rotator cuff surgery ) and I developed adhesive capsulitis, still suffering from constant pain that seems to be unmanageable by any means. 8 days ago, I received a steroid shot behind my shoulder. I felt almost instant relief during day one, but the day after my pain came back worse than prior receiving the shot and one week later I still feeling pain. The pain isn’t in the injection site, it takes my entire shoulder and it increases with every movement ( Having PT is a nightmare! ) and this is making anxious since I had steroid injections in the past, prior to surgery, and I never had this reaction before. I had this shot hoping it would relief my pain and help with my physical therapy,”. Doctor, could you please tell me if having this is normal? I’ll appreciate your response. 😢

Hi Kristy, Unfortunately Stiffness (adhesive capsulitis) after surgery can happen. One way of trying to improve this is a steroid shot. If it didnt work make sure you communicate that to your surgeon. There are other options that you should discuss with your doc. Good Luck Kristy!

I have a salt lamp on in my loungeroom at night to help with light & one in my bedroom so I don't struggle with dark through the night when getting up to go to the toilet or get a drink.

@rotatorcuffexpert it's a lamp with a light n lamp itself is salt its meant to help with damp areas I think? If u can google it it may tell u I use mine more for low light as I have balance probs n get up during the night helps guide me.

Hey, take a dry erase marker and trace the area that you made the mistake on... then wipe it with a wet cloth. Repete process... It will come right off.. I cannot take pain meds due to my digestive issues...🫤😕

Hi Doctor, now it's the 3rd week after the surgery. Physiotherapy exercises cause serve pain and muscle swelling. I am taking opiad pain killers but no relief. Should I stop the exercises ?

Shabir, check with your doctor. PT can be painful after surgery, but its best to check with your surgeon and have an evaluation.
