What is the most common reason for failure you see in car amplifiers? Faulty and/or low quality components, or user error?

95 percent of the time it is user error. Don't get me wrong, today's society is extremely quick to blame the amp and then call it junk but these designs have been around a LONG time. I'm sure if a company or a retailer had even a 10% failure rate they would change something about the design. 10% would be my top failure threshold percentage and not sure about where the manufacturers set theirs.
