Does the mounting plate that attaches to the frame come with the hook or is it sourced elsewhere

Yep it comes with there kit for the tow hook.

It is 8.5 inches from the passenger side of the bumper opening to 12 inches of the passenger opening

When driving the car will the tow hook end up bouncing/scratching off the paint of the lower half of the front bumper that the tow hook sits above like driving on a rough road for example because if I order I don’t wanna have it ruining my paint on my car and damaging the bumper. Thanks
